Indication of Exium 40 IV Injection 

Exium 40 lessens the quantity of acid your stomach produces. Heartburn, acid reflux, and issues with your food pipe are all treated with it. Stomach ulcers can also be prevented and treated with them. Exium 40 should be taken as prescribed by your doctor.

Pharmacology of Exium 40 IV Injection 

By specifically inhibiting the H+/K+ ATPase in the gastric parietal cell, the proton pump inhibitor esomeprazole reduces stomach acid output. The first single optical isomer of a proton pump inhibitor, esomeprazole (S-isomer of omeprazole), offers superior acid control over racemic proton pump inhibitors.

Dosage & Administration

Tablet or Capsule:

• For 4 to 8 weeks, take 20 mg or 40 mg once daily to treat erosive esophagitis. In most cases, recovery takes between 4 and 8 weeks. Patients who do not recover after 4–8 weeks may be offered further 4–8 weeks of treatment.
• 20 mg once daily for the maintenance of erosive esophagitis healing Controlled trials were limited to a six-month period.
• GERD symptoms: 20 mg once each day for 4 weeks. An extra 4 weeks of treatment may be recommended if symptoms do not totally go away after the first 4.
• Esomeprazole 20 mg twice daily for 10 days, Amoxicillin 1000 mg twice daily for 10 days, and Clarithromycin 500 mg twice daily for 10 days are the three components of the triple therapy for the elimination of H. pylori.

Injection: 

• The recommended adult dose in GERD with Erosive Esophagitis is either 20 or 40 mg Esomeprazole is given once daily by intravenous injection (no less than 3 minutes) or intravenous infusion (10 to 30 minutes).
• A solution for intravenous infusion is prepared by first reconstituting the contents of one vial with 5 ml of 0.9% Sodium Chloride and further diluting the resulting solution to a final volume of 50 ml. The resultant concentration after diluting to a final volume of 50 ml is 0.8 mg/ml.
• 20 mg dose: Withdraw 25 ml of the final solution and administer as an intravenous infusion over 10 minutes to 30 minutes.
• 10 mg dose: Withdraw 12.5 ml of the final solution and administer as an intravenous infusion over 10 minutes to 30 minutes.
• Long-term use of Exium 40 can cause weak bones and a deficiency of minerals such as magnesium. Take adequate dietary intake of calcium and magnesium or supplements as prescribed by your doctor.
• Consult your doctor right away if you develop decreased urination, edema (swelling due to fluid retention), lower back pain, nausea, fatigue, and rash or fever. These could be signs of a kidney problem.

Interaction of Exium 40 IV Injection 

Exium appears to be a selective inhibitor of the cytochrome P450 mono-oxygenase system, there may be an effect on hepatic clearance, but there have been no reports to date of clinically relevant interactions. There is some uncertainty over the effect of Esomeprazole on the oral combined contraceptive pill. Physiological changes similar to those found with omeprazole are likely to take place because of the reduction in gastric acid which is likely to influence the bacterial colonization of the stomach and duodenum and also vitamin B12 absorption.

Contraindications

Esomeprazole is contraindicated in those patients who have known hypersensitivity to any other components of the formulation.

Side Effects of Exium 40 IV Injection 

Side effects reported with Exium include:

• Headache
• Dizziness
• Dryness in mouth
• Nausea
• Abdominal pain
• Constipation
• Flatulence
• Diarrhea

Pregnancy and lactation

Pregnancy The S-isomer of omeprazole, esomeprazole, has not been adequately studied in pregnant women, and the available epidemiologic data do not support a link between first-trimester omeprazole use and an increased risk of significant congenital abnormalities or other negative pregnancy outcomes. The dosage-dependent embryo-lethality observed in reproduction tests in rats and rabbits occurred at omeprazole doses that ranged from 3.4 to 34 times the 40 mg oral human dose (based on a body surface area for a 60 kg person) There are no clinical studies on the effects of esomeprazole on breastfed infants or milk supply. Esomeprazole is the S-isomer of omeprazole, and limited evidence indicates that omeprazole may be found in human milk.

Along with the mother's clinical requirement for therapy and any potential negative effects on the breastfed child from treatment or from an underlying maternal ailment, the developmental and health benefits of nursing should be taken into consideration.

Precautions & Warnings

Exclude the possibility of malignancy when gastric ulcer is suspected and before treatment for dyspepsia. When used in combination with antibiotics, refer to the prescribing information of the respective antibiotics.
