✔️ How Escilex 10 ODT Works

Escilex (escitalopram) acts by selectively inhibiting the reuptake of serotonin (5-HT) in the brain, leading to increased availability of serotonin in the synaptic cleft. This modulation of serotonergic activity contributes to mood enhancement and reduction in symptoms of anxiety, panic attacks, and obsessive behaviors. Escitalopram has minimal influence on norepinephrine and dopamine neurotransmission.

✔️ Common Side Effects

Frequently reported (1–10% or more):

• Nausea
• Headache
• Fatigue
• Sleep disturbances (insomnia or somnolence)
• Excessive sweating
• Reduced libido
• Delayed ejaculation
• Orgasmic difficulties in women

Less common (<1%):

• Dizziness, tremors, irritability, drowsiness, abnormal dreams
• Gastrointestinal disturbances (e.g., constipation, flatulence)
• Appetite loss, weight fluctuations
• Blurred vision, dry mouth
• Muscle and joint pain, menstrual irregularities
• Urinary frequency or tract infections
• Anxiety symptoms may transiently worsen during initiation

Rare but serious: Serotonin syndrome, hyponatremia, or increased suicidal ideation, especially in younger patients

✔️ Dosage Guidelines

Adults:

• Depression, Anxiety, OCD: Start with 10 mg once daily; dose can be increased after one week if needed. The maximum recommended dose is 20 mg/day.
• Panic Disorder: Begin with 5 mg once daily, titrating to 10 mg after one week. Maximum dose: 20 mg/day.

Elderly Patients:

• A reduced dose is typically prescribed—half the adult dose is recommended.

Hepatic Impairment:

• Mild to moderate liver impairment: Start at 5 mg/day and may increase to 10 mg/day after two weeks.
• Severe hepatic impairment: Dose titration must be done cautiously.

Pediatric Use:

• Children under 12: Safety and efficacy not established.
• 12 years and older: Start at 10 mg/day, may increase after at least three weeks. Do not exceed 20 mg/day.

Missed Dose

• If a dose is missed, take it as soon as remembered. If it's close to the time of the next scheduled dose, skip the missed one. Do not double the dose to make up for a missed dose.

✔️ Drug Interactions

Major interactions include:

• Increased bleeding risk when used with NSAIDs, aspirin, or anticoagulants.
• Altered plasma levels when taken with:
• CYP2C19 inducers (e.g., carbamazepine, phenytoin, rifampin) → may reduce escitalopram levels.
• CYP2C19 inhibitors (e.g., fluconazole, omeprazole, fluvoxamine) → may increase escitalopram levels.
• CYP3A4 inhibitors/inducers (e.g., azole antifungals, clarithromycin, nafcillin) → variable effects.
• Increased serotonergic toxicity risk with serotonergic agents (e.g., linezolid, sibutramine).
• May enhance CNS depression if taken with alcohol or sedatives.
• Avoid MAOIs—starting escitalopram while on MAOIs or shortly after discontinuing them can lead to serious or fatal reactions.

✔️ Use in Pregnancy and Lactation

• Pregnancy: Use only if benefits clearly outweigh risks. Late pregnancy exposure may cause neonatal complications like respiratory distress, feeding difficulties, and withdrawal symptoms.
• Lactation: Escitalopram is excreted in breast milk. Infants may receive around 3.9% of the maternal weight-adjusted dose. Monitor infants for drowsiness, poor feeding, or irritability.

✔️ Precautions and Warnings

• Use with caution in patients with a history of seizures, mania, bipolar disorder, or suicidal thoughts.
• Exercise caution in patients with liver or kidney impairment.
• May impair judgment or coordination—avoid driving or operating machinery until you know how it affects you.
• Use with extreme caution during pregnancy and lactation.
• Should be tapered gradually when discontinuing to avoid withdrawal effects.
• Not recommended for use with monoamine oxidase inhibitors (MAOIs) due to risk of serotonin syndrome.
