Bisoprolol Fumarate is a highly selective β1-blocker that primarily acts on β1-adrenergic receptors in the heart. It lowers heart rate and cardiac output, which helps reduce blood pressure and the workload of the heart. Unlike some non-selective beta-blockers, long-term treatment with bisoprolol generally does not adversely affect cholesterol fractions, including protective HDL cholesterol.
Bisoprolol is well absorbed after oral administration, with more than 90% absorbed from the gastrointestinal tract. Food does not significantly affect its absorption. It is metabolized mainly in the liver and eliminated through both the kidneys and liver. Its elimination half-life is approximately 10–12 hours, supporting once-daily dosing.

It should not be used for the emergency management of hypertensive crises.
Adults: For mild to moderate hypertension, the usual starting dose is 5 mg once daily, either alone or in combination with a diuretic. If the response is inadequate, the dose may be increased to 10 mg once daily and, when necessary, up to 20 mg once daily. Dose adjustments are generally made at intervals of about two weeks.
Children: The safety and effectiveness of bisoprolol in children have not been established.
Renal or Hepatic Impairment: Patients with significant kidney or liver impairment should generally start with 5 mg once daily. Dose increases should be made cautiously because the medicine may accumulate.
Elderly: Dose adjustment is usually unnecessary unless significant renal or hepatic impairment is present.
Pregnancy: There is limited information regarding the use of bisoprolol in pregnant women. Animal studies have shown fetal effects at certain doses. Therefore, it should only be used during pregnancy when the potential benefit outweighs the possible risk to the fetus.
Lactation: It is not known whether bisoprolol passes into human breast milk. If treatment is considered essential, breastfeeding may need to be discontinued following medical advice.
Tabis should be used cautiously in patients with kidney or liver impairment, and dose adjustments may be necessary. Beta-blockers may also increase sensitivity to allergens in patients with a history of severe allergic reactions, and such patients may respond less effectively to standard doses of epinephrine during a severe allergic reaction.
