✔️ Dosage & Administration of Bisoren 10

✔ Adults (Hypertension & Angina):

• Start with 5 mg once daily (alone or with a diuretic).
• If needed, increase to 10 mg once daily, then to 20 mg once daily.
• Maximum effective dose: 20 mg/day.
• Titrate dose every 2 weeks if needed.

✔ Patients with Renal or Hepatic Impairment:

• Start with 5 mg once daily.
• Use caution with dose increases (risk of drug accumulation).
• Not removed by dialysis—no need for replacement dosing.

✔ Elderly Patients:

• Usually no dose adjustment needed unless severe renal or hepatic dysfunction is present.

🚫 Children:

• Safety and effectiveness not established—not recommended.

✔️ Side Effects of Bisoren 10

✔ Common Side Effects:

• Fatigue & Dizziness
• Headache
• Digestive Issues – Nausea, vomiting, diarrhea, constipation, abdominal pain
• Cold or numb hands and feet
• Muscle weakness or cramps

✔ Less Common but Serious Side Effects:

• Slow heart rate (bradycardia)
• Breathing difficulties (bronchospasm) in asthma or COPD patients
• Severe hypotension (low blood pressure)

✔️ Avoid combining Bisoren with:

• Other β-blockers – Risk of excessive bradycardia or hypotension.
• Catecholamine-depleting drugs (e.g., reserpine, guanethidine) – May lead to excessive suppression of sympathetic activity.
• Centrally Acting Antihypertensives (e.g., Clonidine) – Can cause rebound hypertension when discontinued. If used together:
  • Taper off β-blocker first, then stop clonidine a few days later.
• Antiarrhythmic Agents (e.g., Verapamil, Diltiazem, Disopyramide) – Can cause bradycardia, conduction issues, severe hypotension, or heart failure.
• Calcium Channel Blockers (e.g., Amlodipine, Diltiazem, Verapamil) – Risk of excessive heart rate slowing and low blood pressure.

✔️ Do NOT use Bisoren if you have:

• Cardiogenic shock
• Overt heart failure
• Severe A-V block (2nd or 3rd degree)
• Severe sinus bradycardia
• Right ventricular failure due to pulmonary hypertension

⚠ Pregnancy:

• Bisoprolol was not teratogenic in animal studies, but can be toxic at high doses.

⚠ Lactation:

• Small amounts of Bisoprolol are excreted in animal milk—unknown for humans.
• If essential for the mother, breastfeeding should be stopped.

✔ History of Severe Allergic Reactions – May have reduced response to epinephrine during anaphylaxis.
