লেভ্যামলোডিপিন ম্যালিয়েট প্রাপ্তবয়স্ক এবং ৬ বছর বা তার বেশি বয়সী শিশুদের উচ্চ রক্তচাপের চিকিৎসায় ব্যবহৃত হয়। এটি একটি ডাইহাইড্রোপাইরিডিন ক্যালসিয়াম চ্যানেল ব্লকার। এটি রক্তনালীর মসৃণ পেশিতে ক্যালসিয়াম প্রবেশ বাধাগ্রস্ত করে, ফলে রক্তনালী শিথিল ও প্রসারিত হয় এবং রক্তচাপ কমে।
অ্যামলোডিপিন হলো লেভ্যামলোডিপিন ও ডেক্সট্রো-অ্যামলোডিপিনের ১:১ রেসেমিক মিশ্রণ। এর মধ্যে লেভ্যামলোডিপিন হলো প্রধানত সক্রিয় অ্যান্টিহাইপারটেনসিভ আইসোমার।
শোষণ: মুখে সেবনের পর সাধারণত ৬–১২ ঘণ্টার মধ্যে সর্বোচ্চ রক্তমাত্রায় পৌঁছায়। এর জৈবপ্রাপ্যতা প্রায় ৬৪–৯০% এবং খাবার এর শোষণে উল্লেখযোগ্য প্রভাব ফেলে না।
বিপাক ও নিঃসরণ: ওষুধটির অধিকাংশ লিভারে CYP3A4 এনজাইমের মাধ্যমে নিষ্ক্রিয় মেটাবোলাইটে পরিণত হয়। প্রায় ৬০% প্রস্রাবের মাধ্যমে এবং প্রায় ১০% অপরিবর্তিত অবস্থায় শরীর থেকে বের হয়ে যায়।

Levamlodipine is a calcium channel blocker. It affects the movement of calcium into the cells of the heart and blood vessels. This relaxes the blood vessels and lowers blood pressure. A lower blood pressure will increase the supply of blood and oxygen to the heart.

- CYP3A ইনহিবিটর: কিছু শক্তিশালী বা মাঝারি CYP3A ইনহিবিটরের সঙ্গে ব্যবহার করলে অ্যামলোডিপিনের রক্তমাত্রা বেড়ে যেতে পারে এবং ডোজ কমানোর প্রয়োজন হতে পারে।
- সিমভাস্ট্যাটিন: অ্যামলোডিপিনের সঙ্গে ব্যবহার করলে সিমভাস্ট্যাটিনের মাত্রা বৃদ্ধি পেতে পারে। তাই সিমভাস্ট্যাটিনের দৈনিক ডোজ সাধারণত ২০ মি.গ্রামের মধ্যে সীমাবদ্ধ রাখা হয়।
- সাইক্লোস্পোরিন/ট্যাক্রোলিমাস: অ্যামলোডিপিন এসব ওষুধের রক্তমাত্রা বাড়াতে পারে। প্রয়োজনে চিকিৎসকের পরামর্শে নিয়মিত মাত্রা পর্যবেক্ষণ ও ডোজ সমন্বয় করতে হয়।
- খাবারের সঙ্গে বা খাবার ছাড়াই সেবন করা যায়।
- জাম্বুরা (grapefruit)জাতীয় পণ্য ও প্রাকৃতিক লিকোরিস ব্যবহারের আগে চিকিৎসকের পরামর্শ নেওয়া উচিত।
✔️ মাত্রা ও সেবনবিধি
- সেবন পদ্ধতি: মুখে।
- প্রাপ্তবয়স্কদের সাধারণ প্রাথমিক মাত্রা: ২.৫ মি.গ্রা. দিনে একবার।
- সর্বোচ্চ মাত্রা: ৫ মি.গ্রা. দিনে একবার।
- বয়স্ক ব্যক্তি বা লিভারের সমস্যা থাকলে: ১.২৫ মি.গ্রা. দিনে একবার দিয়ে শুরু করা যেতে পারে।
- ডোজ রোগীর রক্তচাপের লক্ষ্যমাত্রা অনুযায়ী চিকিৎসক সমন্বয় করতে পারেন।
- সাধারণত ডোজ পরিবর্তনের মধ্যে ৭–১৪ দিন সময় দেওয়া হয়।
- ৬–১৭ বছর বয়সী শিশুদের: ১.২৫–২.৫ মি.গ্রা. দিনে একবার ব্যবহার করা যেতে পারে। শিশুদের ক্ষেত্রে ২.৫ মি.গ্রামের বেশি দৈনিক ডোজ পর্যাপ্তভাবে গবেষণা করা হয়নি।
রেজিস্টার্ড চিকিৎসকের পরামর্শ অনুযায়ী ওষুধ সেবন করুন।
অতিরিক্ত মাত্রায় সেবন করলে রক্তনালী অতিরিক্ত প্রসারিত হয়ে তীব্র হাইপোটেনশন (রক্তচাপ কমে যাওয়া) এবং দ্রুত হৃদস্পন্দন হতে পারে। অতিরিক্ত মাত্রা গ্রহণের সন্দেহ হলে দ্রুত চিকিৎসা সহায়তা নিতে হবে।

- গুরুতর করোনারি ধমনীর রোগে আক্রান্ত কিছু রোগীর ক্ষেত্রে ওষুধ শুরু বা ডোজ বাড়ানোর পর এনজাইনা বেড়ে যেতে পারে এবং বিরল ক্ষেত্রে হার্ট অ্যাটাক হতে পারে।
- গুরুতর লিভারের সমস্যা থাকলে ওষুধটি ধীরে ধীরে ডোজ সমন্বয় করে ব্যবহার করতে হয়, কারণ এ ধরনের রোগীর শরীরে ওষুধের অবস্থানকাল দীর্ঘ হতে পারে।
