How does Rocovas 10mg Tablet work?

Rosuvastatin is a lipid-lowering medication (statin). It works by blocking an enzyme (HMG-CoA-reductase) that is required by the body to make cholesterol, thereby lowering "bad" cholesterol (LDL) and triglycerides and raising "good" cholesterol (HDL).

Indications:

Rosuvastatin is prescribed to treat high cholesterol by lowering "bad" cholesterol (LDL) and triglycerides (fats). It should be used in conjunction with regular exercise and a low-fat diet.

Pharmacology

Rosuvastatin is a selective and competitive inhibitor of HMG-CoA reductase, the enzyme responsible for converting 3-hydroxy-3-methyl glutaryl coenzyme A to mevalonate, a precursor of cholesterol. It works by:

1. Increasing hepatic LDL receptors on cell surfaces enhances the uptake and breakdown of LDL.
2. Inhibiting hepatic synthesis of VLDL, thus reducing the total number of VLDL and LDL particles.

Dosage & Administration of Rocovas 10mg Tablet

• General Dose Range: 5-40 mg once daily. The 40 mg dose is reserved for patients not reaching LDL-C goals with 20 mg.
• Homozygous Familial Hypercholesterolemia (HoFH): Starting dose of 20 mg/day.
• Pediatric Patients with Heterozygous Familial Hypercholesterolemia (HeFH):
  • Ages 8 to <10 years: 5-10 mg/day.
  • Ages 10 to 17 years: 5-20 mg/day.
• Pediatric Patients with HoFH (Ages 7 to 17 years): 20 mg/day.
• Note: Always take medication as advised by your doctor.

Administration:

• Rosuvastatin can be taken with or without food, at any time of day.
• Note: Always follow your doctor's advice on medication intake.

Interaction

• Cyclosporine: Increases Rosuvastatin exposure; limit Rosuvastatin to 5 mg once daily.
• Gemfibrozil: Avoid combination; if used together, limit Rosuvastatin to 10 mg once daily.
• Lopinavir/Ritonavir or Atazanavir/Ritonavir: Increases Rosuvastatin exposure; limit Rosuvastatin to 10 mg once daily.
• Coumarin Anticoagulants: Prolongs INR; ensure stable INR before starting Rosuvastatin, monitor INR frequently until stable.
• Concomitant Lipid-Lowering Therapies: Using fibrates and niacin increases the risk of skeletal muscle effects.

Contraindications

• Known hypersensitivity to product components.
• Liver disease, including unexplained persistent elevations in hepatic transaminase levels.
• Pregnant women and women who may become pregnant.
• Nursing mothers.

Pregnancy & Lactation

• Safety in pregnant women has not been established.
• Unknown if excreted in human milk.

Precautions & Warnings

• Skeletal Muscle Effects: Risks increase with 40 mg dose, advanced age (>65 years), hypothyroidism, renal impairment, and combination with cyclosporine, lopinavir/ritonavir, atazanavir/ritonavir, or other lipid-lowering drugs. Report unexplained muscle pain, tenderness, or weakness promptly. Discontinue Rosuvastatin if symptoms appear.
• Liver Enzyme Abnormalities: Monitor liver enzymes before and during treatment.
• Children: Safety and effectiveness not established.
