  • Medical patients with restricted mobility: 40 mg SC once daily; continue until the risk of DVT has significantly reduced (6-11 days) or the patient is on anticoagulant therapy.

Deep Vein Thrombosis (Treatment):

  • Inpatient treatment (with or without pulmonary embolism) when coadministered with warfarin sodium: 1 mg/kg SC every 12 hours or 1.5 mg/kg SC once daily (administer at the same time each day).
  • Outpatient treatment (without pulmonary embolism) when coadministered with warfarin sodium: 1 mg/kg SC every 12 hours.

Unstable Angina & Non-Q-Wave MI:

  • Prophylaxis of ischemic complications when concurrently administered with aspirin: 1 mg/kg SC every 12 hours.

Acute ST-Segment Elevation Myocardial Infarction (STEMI):

  • Loading dose: 30 mg IV bolus once, plus 1 mg/kg SC once (not to exceed 100 mg cumulative loading dose).
  • Maintenance dose for patients <75 years: 1 mg/kg SC every 12 hours.
  • Maintenance dose for patients >75 years: 0.75 mg/kg SC every 12 hours (no initial IV bolus).

Renal Dose:
