✔️ Pharmacology

Nabumetone, the active component in Nabumax, is a non-steroidal anti-inflammatory drug (NSAID) with anti-inflammatory, analgesic, and antipyretic effects. Its exact mechanism is unknown, but it is thought to inhibit prostaglandin synthesis. Nabumetone is a prodrug that is metabolized in the liver to 6-methoxy-2-naphthylacetic acid (6MNA), which is responsible for its therapeutic effects.

✔️ Dosage & Administration of Elivier DT

• Osteoarthritis and Rheumatoid Arthritis: Start with 1000 mg once daily, with or without food. Some patients may benefit from 1500 mg to 2000 mg daily. Nabumetone can be taken once or twice daily. Dosages above 2000 mg/day have not been studied. Use the lowest effective dose for long-term treatment.
• Renal Impairment: In patients with moderate renal insufficiency, limit the dose to 750 mg once daily. In severe renal impairment, start with 500 mg once daily, monitoring renal function. Maximum doses: 1500 mg (moderate insufficiency), 1000 mg (severe insufficiency).
• Pediatric Use: Safety and efficacy in children have not been established.

NSAIDs may reduce the effectiveness of ACE inhibitors. Consider this interaction when using NSAIDs alongside ACE inhibitors.

Long-term NSAID use has been linked to renal papillary necrosis and renal toxicity, especially in patients with compromised renal function, heart failure, liver dysfunction, or those taking diuretics. NSAIDs reduce renal blood flow by decreasing prostaglandin synthesis, which can cause renal decompensation. Elderly patients are at higher risk, and discontinuing NSAID therapy typically restores renal function.

Symptoms of overdose include lethargy, nausea, vomiting, drowsiness, and gastrointestinal bleeding. More severe reactions like hypertension, renal failure, and respiratory depression are rare. Overdose management involves supportive care, emesis, activated charcoal, and monitoring of renal and gastrointestinal function. Hemodialysis is generally not helpful due to high protein binding.
