Arthanib XR is used to manage moderate to severe rheumatoid arthritis (RA) in adults. It helps reduce joint pain, tenderness, swelling, and inflammation while slowing the progression of joint and bone damage. It may be used alone or together with methotrexate or other non-biological disease-modifying antirheumatic drugs (DMARDs) when previous treatment has not provided adequate relief.
Arthanib XR may be taken with or without food. Take it exactly as prescribed by your doctor, preferably at the same time each day. Swallow the tablet whole with water and do not crush, chew, or break it. Do not discontinue treatment without medical advice, even if you start feeling better.
Adult – Rheumatoid Arthritis:
The usual recommended dose of immediate-release tofacitinib for rheumatoid arthritis is 5 mg twice daily. The dose may need to be reduced in patients with moderate to severe kidney impairment or moderate liver impairment. It is not recommended in severe hepatic impairment.
Children:
The safety and effectiveness of tofacitinib for rheumatoid arthritis in children under 18 years have not been established.
Take Arthanib XR in the dose and for the duration prescribed by your doctor. It can be taken with or without food, but taking it at a consistent time each day may help you remember your dose. Swallow the tablet whole; do not chew, crush, or break it.
Some medicines can alter the amount of tofacitinib in the body. Strong CYP3A4 inhibitors, such as ketoconazole, may increase tofacitinib exposure, while strong CYP3A4 inducers such as rifampicin may reduce its effectiveness. Fluconazole may also increase tofacitinib exposure.
Tacrolimus and cyclosporine may increase exposure to tofacitinib and should be used with caution. Tofacitinib may also affect exposure to methotrexate.
Always inform your doctor about all prescription medicines, over-the-counter products, supplements, and herbal medicines you are taking before starting Arthanib XR.
Arthanib XR is not generally recommended during pregnancy because available human data are limited and animal studies suggest potential risks to the developing fetus. Women who can become pregnant should use effective contraception during treatment and for at least 4 weeks after the final dose.
It is not known whether tofacitinib passes into human breast milk. Breastfeeding is therefore not recommended during treatment. Discuss with your doctor whether to stop breastfeeding or discontinue treatment.
Arthanib XR should not be used in patients with a known allergy to tofacitinib or any of the ingredients in the medicine. It should generally not be initiated in patients with an active serious infection.
Tofacitinib can increase the risk of serious infections, including tuberculosis and opportunistic infections. Patients should be evaluated for active or latent infections before treatment begins. Treatment should be interrupted if a serious infection develops until it is adequately controlled.
The medicine may also be associated with changes in blood cell counts, liver enzymes, and lipid levels. Your doctor may therefore recommend regular laboratory monitoring.
Patients with a history of recurrent or serious infections, tuberculosis exposure, viral hepatitis, malignancy, diverticulitis, or gastrointestinal problems should inform their doctor before starting treatment.
Tofacitinib may increase the risk of blood clots and certain malignancies. Patients at increased risk of skin cancer may require periodic skin examinations.
Arthanib XR should generally not be combined with biological DMARDs or other potent immunosuppressive medicines unless specifically directed by a specialist.
Store Arthanib XR tablets at controlled room temperature
