Indication of Tofanib 5mg Tablet

Tofacitinib is indicated for the treatment of adult patients with moderate to severely active rheumatoid arthritis who have not responded or are intolerant to methotrexate. It can be used alone or in combination with methotrexate or other non-biologically modifying anti-rheumatic medications (DMARDs).

Tofacitinib is indicated for the treatment of adult patients with active psoriatic arthritis who have a poor response or intolerance to methotrexate or other disease-modifying antirheumatic drugs (DMARD).

Tofacitinib is approved for the treatment of adult patients with moderate to severe active ulcerative colitis (UC).

Pharmacology

Tofacitinib is a JAK inhibitor that is partially reversible and prevents the body from responding to cytokine signals. Tofacitinib prevents STAT phosphorylation and activation by inhibiting JAK. The JAK-STAT signaling pathway regulates cell transcription in hematopoietic and immune cell functions. Tofacitinib works as an anti-inflammatory agent by inhibiting the JAK-STAT pathway. However, there is evidence that it can also have other healing effects.

Rheumatoid arthritis is an autoimmune disease characterized by pro-inflammatory cytokine dysregulation, including IL7, IL15, IL21, IL6, IFN, and IFN. (3) Through the Janus kinase signaling pathway, cytokine signaling promotes the recruitment and activation of immune cells, resulting in tissue inflammation and joint damage.

Dosage & Administration of Tofanib 5mg Tablet

• Tofacitinib 5mg twice daily or Tofacitinib 11 mg once daily for rheumatoid arthritis. Tofacitinib 5 mg once daily is recommended in patients with moderate to severe renal impairment or moderate Hepatic Impairment.
• Tofacitinib 5mg twice daily or Tofacitinib 11mg once daily for Psoriatic Arthritis (in combination with nonbiologic DMARDs). Tofacitinib 5mg once daily is recommended in patients with moderate to severe Renal Impairment or moderate hepatic impairment.
• Tofacitinib 10mg twice daily for at least 8 weeks, followed by 5 or 10mg twice daily. If an adequate therapeutic benefit is not obtained after 16 weeks of 10 mg twice daily, discontinue. To maintain response, use the lowest effective dose.

Interaction

• Prior to ingesting Tofanib 5mg Tablet If you are taking any of the following medications, let your doctor know:
• Rifampicin and other antibiotics (used in treating bacterial infections)
  Ketoconazole with fluconazole (used to treat fungal infections)
  Tacrolimus, mercaptopurine, ciclosporin, and azathioprine (medicines that depress the immune system)
• The use of corticosteroids like prednisone (used in treating pain and inflammation)

Overdosage of Tofanib 5mg Tablet

If you or anyone else unintentionally consumes too much Tofanib 5mg Tablet, call your doctor right away or go to the hospital in the area.

Contraindications

Hypersensitivity to any of the components of the formulation.

Pregnancy & Lactation

Pregnancy category C. There are no adequate and well-controlled studies of pregnant women. Tofacitinib should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether tofacitinib is excreted in human milk. Since many drugs are excreted in human milk, and because tofacitinib may have serious adverse reactions to nursing babies, it is necessary to decide whether to stop breastfeeding or the drug, taking into account the importance of the drug for breastfeeding. 

Precautions & Warnings

• Before starting therapy with Tofanib 5mg Tablet, after 4–8 weeks of treatment, and then every three months, your doctor could advise getting a blood test done to find out your blood cell count.
• Your doctor may recommend routine testing to check your liver and blood cholesterol levels.
• If you develop breathing problems or observe painful skin blisters, let your doctor know.
• Use effective contraception while taking Tofanib 5mg Tablet and for 4 weeks after stopping if you are a woman of childbearing potential.
• If you have serious infections, bloodstream infections, active TB, or severe liver issues like cirrhosis, you should not use Tofanib 5mg Tablet.
• You should also avoid taking it if you have allergies to any of its ingredients.
• If you are trying to get pregnant or are nursing, stay away from Tofanib 5mg Tablet.
• Inform your doctor if you have an infection, symptoms of an infection, a condition that increases your risk of infection, are taking any immune system-altering medications, have or had a history of tuberculosis, liver issues, hepatitis B or C, cancer, are at a high risk of developing skin cancer, inflammation/ulcers of the large intestine, kidney issues, plan to get vaccinated, or have hepatitis A or B, high cholesterol, hypertension, heart stroke, diabetes, or issues with blood clotting.
• Avoid using Tofanib 5mg Tablet if you have low red or white blood cell counts.
• Children under the age of 18 should not use Tofanib 5mg Tablet since its safety and effectiveness have not been proven.
