The main mechanism of action of the dopamine antagonist domperidone is the blockage of dopamine receptors in the stomach's chemoreceptor trigger Zone (CTZ). Its capacity to block dopamine receptors, which control gastrointestinal motility, underlies its gastroprokinetic effect. Due to its limited blood-brain barrier penetration, domperidone has essentially little effect on dopaminergic receptors in the brain while excluding any neurologic and psychotropic side effects. Domperidone improves antral and duodenal peristalsis, reduces pylorus contraction, and restores normal upper gastrointestinal motility and tone. Domperidone also improves esophageal peristalsis and lowers esophageal sphincter pressure, which stops the regurgitation of stomach contents.
