How to administer Silvadazin 25gm Cream
  • Burns: Clean the burn wound and apply the cream over all affected areas to a depth of 3-5mm. Reapply as necessary, at least every 24 hours or more frequently if there is significant exudate.
  • Hand Burns: Enclose the whole hand in a clear plastic bag or glove before applying the cream. Change the dressing when excessive exudate accumulates.
  • Leg Ulcers/Pressure Sores: Fill the cavity of the ulcer with cream to a depth of at least 3-5mm. Follow with an absorbent pad or gauze dressing and apply pressure bandaging as appropriate. Change dressings daily or every 48 hours for less exudative wounds.

The antibacterial mechanism of Silver Sulfadiazine involves several steps:

  • Structural Changes in Bacterial Cell Membrane: Upon exposure to the drug, the bacterial cell membrane undergoes structural changes, including distortion, enlargement, and weakening of the cell wall membrane. These alterations contribute to a reduction in the viability of susceptible bacterial strains.
  • Binding of Silver Moiety to Bacterial Cells: The Silver Sulfadiazine molecule dissociates, and the silver component binds to the bacterial cells. This binding process is believed to occur after the silver moiety penetrates the bacterial cell wall.
  • Inhibition of Bacterial Cell Proliferation: Once attached to the bacterial DNA, the silver moiety prevents bacterial cell proliferation. This action disrupts the bacterial replication process, hindering the spread and growth of bacteria.
  • Selective Action on Bacterial DNA: The concentration of DNA in mammalian cells is significantly higher than in bacterial cells. The ratio of silver sulfadiazine to bacterial DNA is thought to be sufficiently high to inhibit bacterial division while maintaining a low enough ratio to prevent interference with epithelial cell regeneration.

Dosage & Administration

Adult Dose

For topical/cutaneous treatment and prophylaxis of infection in severe burns:

  • Apply onto the affected area 1-2 times/day.

Child Dose

  • <2 months: Contraindicated
  • >2 months (1% cream): Apply onto the affected area 1-2 times/day.
Interaction

Drug-Drug Interactions:

  1. Anti-diabetics: Silver Sulfadiazine may potentiate the antidiabetic effect of sulphonylureas. Close monitoring of blood glucose levels is advised when using Silver Sulfadiazine concurrently with anti-diabetic medications.
  2. Local Anaesthetics: Silver Sulfadiazine may interact with various local anesthetics such as lidocaine, causing potential additive effects on nerve function or skin irritation. Caution is recommended when using these medications together.
  3. Antiepileptic Drugs (Phenytoin): Concurrent use of Silver Sulfadiazine with phenytoin, an antiepileptic medication, may potentiate phenytoin's action. Monitoring of phenytoin levels in the blood is advisable in patients receiving both medications.
  4. Anti-ulcer Drugs (Cimetidine): Coadministration of cimetidine, an anti-ulcer medication, with Silver Sulfadiazine, may increase the incidence of leukopenia (reduced white blood cell count) in individuals with large-area burns. Close monitoring of blood cell counts is recommended in such cases.
  5. Enzymes (Collagenase): Silver Sulfadiazine may potentially inactivate enzymatic debriding agents such as collagenase. Avoid concurrent use of these medications due to the risk of reduced efficacy.

Drug-Food Interaction: No significant interactions between Silver Sulfadiazine and food have been identified or established.

Drug-Disease Interactions: Before initiating treatment with Silver Sulfadiazine, inform your doctor if you have any of the following conditions:

  1. Allergic reactions to Silver Sulfadiazine: Patients with a history of hypersensitivity or allergic reactions to Silver Sulfadiazine should avoid its use.
