Silver Sulfadiazine has broad-spectrum antibacterial activity. Its exact mechanism is not fully defined, but evidence suggests:
Structural changes in bacterial cell membranes and weakening of the cell wall.
The silver moiety binds to bacterial DNA, preventing replication and proliferation.
The sulfadiazine moiety contributes a bacteriostatic effect.
Selectivity is maintained because mammalian cells contain higher DNA content, reducing the risk of impaired epithelial regeneration.
Absorption of sulfadiazine through burn wounds can occur, especially in extensive burns; up to 10% may be absorbed in adults and excreted in urine.
Common/Reported: Transient leukopenia.
Less frequent: Skin necrosis, erythema multiforme, skin discoloration, burning sensation, rash, interstitial nephritis.
Cleanse burn wounds thoroughly before application.
Apply Sibalyn Cream once or twice daily in a layer about 1.5 mm (1/16 inch) thick.
Keep burn areas covered at all times; reapply if the cream is removed by movement, dressing changes, or hydrotherapy.
Continue treatment until complete healing or readiness for skin grafting.
Do not discontinue prematurely if there is still risk of infection.
Enzymatic debriding agents: May be inactivated by silver.
Oral hypoglycemics & phenytoin: Systemic absorption of sulfadiazine in extensive burns may potentiate their effects. Monitor blood levels.
Cimetidine: May increase risk of leukopenia in large burn cases.
Known hypersensitivity to silver sulfadiazine or excipients.
Pregnant women near term.
Premature infants and neonates under 2 months (risk of kernicterus).
Pregnancy Category B. Use only if clearly necessary. Avoid near-term.
Unknown if excreted in breast milk. Weigh maternal benefit against risk to the infant.
Safety in pediatric patients has not been firmly established.
Monitor renal and hepatic function in patients with extensive burns (risk of systemic accumulation).
Serum sulfa concentrations should be checked in large-area burn treatment.
Monitor urine for sulfa crystals.
Silver may inactivate proteolytic enzymes used in wound care.
Store below 30°C, protected from light and moisture.
