✅ Description:

Ancor 5mg Tablet is a selective β1-blocker used for managing stable heart failure, hypertension, and angina pectoris. Ancor 5mg Tablet is indicated for:

  • Stable Heart Failure: Used in combination with other medications for patients whose heart failure is controlled and show no symptoms of worsening.
  • Hypertension (High Blood Pressure): Helps reduce persistently elevated arterial blood pressure.
  • Angina Pectoris (Chest Pain): Used to relieve chest pain caused by reduced blood flow to the heart, associated with coronary artery disease.

Bisoprolol selectively blocks β1-adrenergic receptors, primarily affecting the heart. This reduces heart rate and cardiac output, leading to decreased arterial blood pressure. It also lowers the workload on the heart and improves symptoms of angina by reducing oxygen demand.

Do not use Ancor 5mg Tablet in patients with:

  • Severe heart or lung diseases
  • Peripheral arterial occlusive disease
  • Raynaud’s syndrome
  • Untreated pheochromocytoma
  • Metabolic acidosis
  • Hypotension (low blood pressure)

Before taking Ancor 5mg Tablet, inform your doctor if you have:

  • Severe kidney or liver diseases
  • Diabetes
  • Are you on strict fasting or undergoing desensitization therapy
  • History of psoriasis or thyroid disorder
  • Plan to undergo general anesthesia

✔️ Pharmacology

Bisoprolol Fumarate is a highly selective β1-blocker with minimal effects on β2 receptors. It works by selectively blocking β1-adrenergic receptors in the heart and vascular smooth muscle, reducing heart rate and cardiac output, thus lowering arterial hypertension. Unlike non-β1 selective blockers, Bisoprolol does not adversely affect lipid metabolism or reduce cardioprotective HDL-cholesterol in long-term therapy.

Key Pharmacokinetic Properties:

  • Absorption and Bioavailability: Bisoprolol is over 90% absorbed from the gastrointestinal tract with an absolute bioavailability of 88%. Food does not affect its absorption.
  • Distribution: Extensively distributed with a mean distribution volume of 3.5 L/kg.
  • Metabolism: Metabolized via oxidative pathways, primarily by CYP3A4, with minor involvement of CYP2D6. Metabolites are renally excreted and pharmacologically inactive.
  • Elimination: Equally cleared via renal excretion and hepatic metabolism. The elimination half-life is 10-12 hours.
✔️ Dosage & Administration of Ancor 5mg Tablet

Adults:

  • Initial Dose: 5 mg once daily, can be added to a diuretic or used alone.
  • Titration: If 5 mg is inadequate, increase to 10 mg, and if necessary, to 20 mg once daily. Dosage increases should occur at 2-week intervals.
  • Max Dose: Generally, doses beyond 20 mg once daily provide minimal additional benefit.

Children: Safety and effectiveness have not been established.

Renal or Hepatic Impairment:

  • Initial Dose: 5 mg daily. Use caution in dose titration due to possible drug accumulation.
  • Dialysis: Bisoprolol is not dialyzable; thus, drug replacement is not required.

Geriatrics: No dose adjustment is usually necessary unless significant renal or hepatic dysfunction is present.

Note: Always follow the prescribed regimen by a healthcare provider.

✔️ Interaction
  • Other β-Blocking Agents: Do not combine with other β-blockers.
  • Catecholamine-Depleting Drugs: Monitor closely as bisoprolol may excessively reduce sympathetic activity.
  • Centrally Active Antihypertensive Agents: Gradually withdraw β-blockers before discontinuing clonidine to avoid rebound hypertension.
  • Antiarrhythmic Agents: Use with caution when combined with myocardial depressants or inhibitors of A-V conduction (e.g., verapamil, diltiazem, disopyramide).
  • Calcium Channel Blockers: Combined use can prolong S-A and A-V conduction, leading to severe hypotension, bradycardia, and cardiac failure.
✔️ Contraindications
  • Cardiogenic Shock
  • Overt Heart Failure
  • Second or Third-Degree A-V Block
  • Right Ventricular Failure secondary to Pulmonary Hypertension
  • Sinus Bradycardia
✔️ Pregnancy & Lactation
  • Pregnancy: Animal studies showed no teratogenic effects, but there were some fetotoxic and maternotoxic effects at high doses. Use during pregnancy only if the potential benefit justifies the risk.
  • Lactation: Small amounts detected in animal milk; unknown if excreted in human milk. If necessary, mothers should stop nursing.
✔️ Precautions & Warnings
  • Renal or Hepatic Impairment: Adjust doses cautiously.
  • Severe Anaphylactic Reactions: Patients may be more reactive and less responsive to usual doses of epinephrine.
  • Hyperoxaluria: Use vitamin C with caution.
  • Anticoagulant Therapy: Vitamin E may enhance the anticoagulant effect.
✔️ Storage Conditions:

Store in a dry place below 30˚C. Protect from light and heat. Keep out of reach of children.
