Lopirel 75mg Tablet 1 Strip

✔️ Lopirel tablets are prescribed for the prevention of myocardial infarction (MI) and stroke in patients with certain cardiovascular conditions:

Acute Coronary Syndrome (ACS):

• Used to lower the risk of myocardial infarction and stroke in individuals diagnosed with non-ST-segment elevation ACS, including unstable angina (UA) and non-ST-elevation myocardial infarction (NSTEMI).
• Also indicated for reducing the risk of MI and stroke in patients with acute ST-elevation myocardial infarction (STEMI).

Recent MI, Recent Stroke, or Established Peripheral Arterial Disease:

• Administered to patients with a history of recent MI, stroke, or existing peripheral arterial disease to minimize the likelihood of future cardiovascular events.

Note: This medication should be taken under the supervision of a registered physician.

✔️ Dosage & Administration

For Acute Coronary Syndrome (ACS):

• To achieve a rapid antiplatelet effect, treatment begins with a single 300 mg oral loading dose (equivalent to 4 tablets).
• Following this, the patient should continue with a 75 mg daily dose.
• If no loading dose is given, the full antiplatelet effect will be delayed by a few days.

For Recent MI, Stroke, or Peripheral Arterial Disease:

• The standard dosage is 75 mg once daily, with no loading dose required.
• Mode of Administration:
  • Lopirel is taken orally and can be administered with or without food.
• Pediatric Patients:
  • Safety and effectiveness have not been established.
• Elderly Patients:
  • No dosage adjustment is required for elderly individuals.

✔️ Lopirel is generally well tolerated, but some common side effects include:

• Bleeding complications
• Gastrointestinal discomfort (e.g., diarrhea)
• Skin reactions

Rare side effects:

• Anemia, angioedema, arthralgia, arthritis
• Bone marrow disorders, acquired hemophilia

✔️ Drug Interactions

• Increased Bleeding Risk:
  • When taken with NSAIDs, warfarin, or serotonin/norepinephrine reuptake inhibitors (SSRIs, SNRIs), there is a higher risk of bleeding complications.
• Metabolism Inhibition:
  • Drugs like omeprazole and esomeprazole (CYP2C19 inhibitors) can interfere with Clopidogrel’s activation, reducing its effectiveness.
• Repaglinide Interaction:
  • Avoid co-administration, as Clopidogrel increases the plasma concentration of Repaglinide, which may lead to adverse effects.

✔️ Lopirel should not be used in patients with:

• Hypersensitivity to Clopidogrel or any of its components.
• Active bleeding disorders, including peptic ulcers or intracranial hemorrhage.

✔️ Pregnancy & Lactation

• Pregnancy: No adequate studies are confirming the safety of Clopidogrel in pregnant women. It should only be used if the potential benefits outweigh the risks.
• Lactation: It is unknown whether Clopidogrel is excreted in human breast milk. Women should discuss with their physician whether to discontinue the drug or breastfeeding.

✔️ Precautions & Warnings

• Genetic Influence:
  • Since Clopidogrel requires metabolism via CYP2C19, genetic variations (poor metabolizers) or drug interactions with CYP2C19 inhibitors (such as omeprazole) may reduce its efficacy.
• Bleeding Risk:
  • Since Clopidogrel inhibits platelet function for the platelet’s lifespan (7–10 days), the risk of bleeding is elevated.
  • Platelet transfusions within 4 hours of a loading dose or 2 hours of a maintenance dose may be ineffective.
• Surgery Considerations:
  • Discontinuation of Lopirel increases cardiovascular risk, but it should be stopped 5 days before major surgery to reduce excessive bleeding risks.
  • Restart as soon as hemostasis is achieved post-surgery.
• Thrombotic Thrombocytopenic Purpura (TTP):
  • Cases of TTP have been reported, requiring urgent plasma exchange therapy.
• Hypersensitivity:
  • Patients who previously experienced allergic reactions (rash, angioedema, hematologic issues) to Clopidogrel or other thienopyridines should be closely monitored.

✔️ Storage Conditions

• Store at a temperature below 30°C in a dry place, away from moisture and direct light.
• Do not freeze.
• Keep out of reach of children.
