Indication

Opton 20mg Tablet is an anti-ulcer medicine. It is used to treat stomach and intestinal conditions like peptic ulcers (sores in the lining of your stomach and intestine), esophagitis (swelling of your food pipe), and gastroesophageal reflux disease (a condition in which the stomach acid flows back into your food pipe), etc. It also relieves stress ulcers (sores in the stomach and intestine that may occur due to illness, infections, etc.) and acidity caused due to the intake of painkillers. It works by stopping the excess production of acid in your stomach.

Pharmacology

Esomeprazole, the active ingredient in Opton 20mg Capsule, works by preventing the final step of gastric acid production. It achieves this by binding covalently to the cysteine sulfhydryl group in the H+/K+ ATPase enzyme on the secretory surface of gastric parietal cells. This binding inhibits the production of gastric acid.

The inhibitory effect of esomeprazole on gastric acid secretion is not dependent on the specific stimulus. It effectively reduces both basal and stimulated acid secretion. Since the binding of esomeprazole to the H+/K+ ATPase enzyme is irreversible, new enzymes need to be synthesized to restore acid secretion. This means that the antisecretory effect of esomeprazole lasts for more than 24 hours.

• GERD Without Erosive Esophagitis: 20 mg orally once daily for 4 weeks. Consider an additional 4 weeks of treatment if symptoms do not completely resolve in the first 4 weeks.
• GERD With Erosive Esophagitis: 20-40 mg orally once daily for 4-8 weeks. If oral therapy is not possible, intravenous administration of 20-40 mg once daily for up to 10 days can be considered. Switch to oral therapy once the patient is able to swallow.
• Maintenance Therapy: 20 mg orally once daily for up to 6 months.
• Risk Reduction of NSAID-Associated Gastric Ulcer: 20-40 mg orally once daily for up to 6 months.
• NSAID-Induced Gastric Ulcer: 20 mg orally once daily for 4-8 weeks.
• Zollinger-Ellison Syndrome: The initial dose is 80 mg orally divided every 12 hours. Adjust the regimen based on efficacy, up to a maximum of 240 mg orally once daily or 120 mg orally every 12 hours.

Child Dose:

GERD Without Erosive Esophagitis: Safety and efficacy not established for children below 1 year. For children 1-12 years old, the dose is 10-20 mg orally once daily for up to 8 weeks. For children above 12 years, the dose is 20-40 mg orally once daily for up to 8 weeks.

GERD With Erosive Esophagitis (Healing): Safety and efficacy not established for infants below 1 month. For infants 1 month to 1 year (weight-based dosing):

• <3.5 kg: 2.5 mg orally once daily for up to 6 weeks.

3.5-7.5 kg: 5 mg orally once daily for up to 6 weeks.

7.5 kg: 10 mg orally once daily for up to 6 weeks. For children 1-12 years old, the dose is 10 mg orally once daily for 8 weeks if weight is <20 kg, and 10-20 mg orally once daily for 8 weeks if weight is >20 kg. For children above 12 years, the dose is 20-40 mg orally once daily for 4-8 weeks.

Maintenance Therapy: 20 mg orally once daily for up to 6 months.

Renal Dose: No dosage adjustment is needed for renal impairment.
