✔️ Missed Dose + Dosage & Administration of Itra Nova Itraconazole

• Take exactly as prescribed by a physician
• Swallow capsules whole; do not crush or chew
• Should be taken with food, preferably immediately after a full meal
• Dosage and duration vary depending on the infection and may involve cyclic therapy

Examples of Adult Dosing:

• Oropharyngeal candidiasis: 100 mg daily for 15 days
• Vulvovaginal candidiasis: 200 mg twice daily for 1 day
• Pityriasis versicolor: 200 mg daily for 7 days
• Tinea infections: 100 mg daily for 15 days
• Nail infections: 200 mg daily for up to 3 months
• Systemic infections: 100–200 mg once or twice daily

Pediatric Use:

• 5–10 mg/kg/day depending on condition (max 200 mg/day)
• Not recommended for children under 3 years (safety not established)

If a dose is missed, take it as soon as remembered. If it is near the time for the next dose, skip the missed dose and continue as scheduled. Do not double the dose.

✔️ Side Effects

• Nausea
• Headache
• Cough and cold symptoms
• Menstrual irregularities
• Blurred vision

Others:

• Abdominal pain, diarrhea, constipation
• Rash, itching
• Dizziness, fatigue
• Abnormal liver function tests

Serious (Rare but Important):

• Liver failure
• Congestive heart failure
• Pulmonary edema

✔️ Special Population

• Pregnancy: Avoid unless the benefit outweighs the risk
• Lactation: Excreted in breast milk—use with caution
• Elderly: Use carefully, especially with organ impairment
• Renal impairment: Dose adjustment may be required
• Hepatic impairment: Monitor closely

✔️ Drug Interaction

• Hypersensitivity to azole antifungals
• Pregnancy and lactation (unless clearly necessary)
• Active liver disease
• Severe renal impairment (for IV form)

✔️ Itraconazole has significant interaction potential:

• Increases levels of: anticoagulants, digoxin, corticosteroids, benzodiazepines, some antidiabetics
• Affected by: rifampicin, carbamazepine, phenytoin (reduce efficacy)
• Reduced absorption with: antacids, PPIs, H2 blockers
• Avoid with QT-prolonging drugs (risk of serious arrhythmias)
• May increase risk of muscle toxicity with statins (e.g., atorvastatin)

✔️ Precautions & Warnings

• Monitor liver function, especially during prolonged therapy
• Use cautiously in patients with:
  • Heart failure or history of heart disease
  • Liver or kidney impairment
  • Weakened immune system (e.g., HIV/AIDS)
• Avoid antacids within 1–2 hours of dosing
• May cause dizziness or blurred vision—avoid driving if affected

Serious Warning Signs (seek medical attention):

• Symptoms of liver damage (yellowing of skin/eyes, dark urine)
• Signs of heart failure (shortness of breath, swelling, rapid weight gain)
• Severe allergic reactions (rash, swelling, breathing difficulty)
