✔️ Pharmacology

• The active form of Vitamin D3 is Calcitriol, which binds to Vitamin D receptors (VDRs) in various tissues.
• Absorbed in the small intestine, Vitamin D3 is bound to specific a-globulins and transported to the liver where it is converted to 25-hydroxy Vitamin D3 (Calcidiol). Further hydroxylation occurs in the kidney to form 1,25-dihydroxy Vitamin D3 (Calcitriol), which is responsible for increasing calcium absorption.
• Elimination: Excreted via feces and urine, with non-metabolized Vitamin D3 stored in tissues like fat and muscle.
• Half-Life: Approximately 50 days, being fat-soluble.

✔️ Dosage of Calciferol IM Injection

• Prevention:
  • Infants with Vitamin D Enriched Milk: ½ ampoule (0.5 ml, 100,000 IU) every 6 months.
  • Other Infants & Young Children: 1 ampoule (1 ml, 200,000 IU) every 6 months.
  • Adolescents: 1 ampoule (1 ml, 200,000 IU) every 6 months during winter.
  • Pregnancy: ½ ampoule (0.5 ml, 100,000 IU) from the 6th or 7th month of pregnancy.
  • Elderly: ½ ampoule (0.5 ml, 100,000 IU) every 3 months.
  • Special Conditions: ½ or 1 ampoule (100,000-200,000 IU) every 3-6 months.
• Deficiency: 1 ampoule (1 ml, 200,000 IU) can be repeated after 1-6 months.

✔️ Before taking Calciferol IM Injection, inform your doctor, if you are taking any of the following medicines:

• Medicines used to manage epilepsy (Ex. carbamazepine, phenobarbital, phenytoin, primidone)
• Other vitamin D-containing medicines including multi-vitamins
• Medicines to control the rate of your heartbeat (Ex. digoxin, digitoxin)
• Diuretics (water tablets) (Ex. Bendroflumethiazide)
• Calcium supplements
• Medicines used to manage tuberculosis (Ex. rifampicin, isoniazid)
• Medicines leading to fat malabsorption (Ex. orlistat, cholestyramine, liquid paraffin)
• Medicines used to manage fungal infections (Ex. ketoconazole, itraconazole)
• Actinomycin (medicine used to manage some forms of cancer)
• Glucocorticosteroids Ex. hydrocortisone or prednisolone (used to manage inflammatory conditions)

✔️ Contraindications

• Hypercalcemia: Contraindicated in all conditions with high calcium levels.
• Hypersensitivity: Avoid in patients with known hypersensitivity to Vitamin D3 or its components.
• Vitamin D3 Toxicity: Should not be used if there is evidence of toxicity.

✔️ Side Effects

• Regular plasma-calcium concentration monitoring is required in high doses, renal impairment, and during pregnancy & lactation.
• Consult a healthcare provider if using Digoxin, Thiazide Diuretics, or in cases of liver or kidney disease, hyperthyroidism, lymphoma, tuberculosis, and granulomatous disease.
