Indication of Bigmet 850mg Tablet

Bigmet 850mg Tablet is a medication used to control high blood sugar levels in people with type 2 diabetes. Type 2 diabetes is a condition where the body either does not produce enough insulin or is resistant to insulin. Insulin is a hormone that helps convert glucose from food into energy. Bigmet 850mg Tablet is effective in reducing blood glucose levels in both adults and children with type 2 diabetes.

In addition to diabetes, Bigmet 850mg Tablet is also used in the treatment of Polycystic Ovary Syndrome (PCOS), a hormonal disorder that affects women of reproductive age. PCOS is characterized by the enlargement of ovaries with cysts on the outer lining, often leading to hormonal imbalances and irregular menstrual cycles.

Bigmet 850mg Tablet is a valuable medication for managing blood sugar levels and can be beneficial for individuals with type 2 diabetes and PCOS. It is important to use this medication as prescribed by a healthcare professional and to follow a healthy lifestyle, including diet and exercise, to effectively manage these conditions.

Pharmacology

Metformin is an antihyperglycemic medication belonging to the biguanide class. It is used to treat type 2 diabetes. Metformin works differently from sulfonylureas and does not cause hypoglycemia. It helps to lower both baseline and postprandial (after-meal) plasma glucose levels.

The mode of action of metformin involves several mechanisms. It enhances insulin sensitivity, which means it improves the body's response to insulin, leading to better glucose uptake and utilization by peripheral tissues. Additionally, metformin reduces glucose production by the liver and lowers glucose absorption from the intestines.

Overall, metformin is effective in improving glycemic control in patients with type 2 diabetes by targeting various pathways involved in glucose metabolism without causing episodes of low blood sugar levels (hypoglycemia). It is an important medication in the management of type 2 diabetes and is commonly used as a first-line therapy in combination with lifestyle changes such as diet and exercise.

Dosage & Administration of Bigmet 850mg Tablet

Adult Dose: For type 2 diabetes mellitus, the conventional preparation of Bigmet 850mg Tablet is usually started with 500 mg taken twice or thrice daily, or 850 mg taken once or twice daily. The dose may be gradually increased to a maximum of 2000 mg per day at weekly intervals. For the modified-release preparation, the initial dose is 500 mg once daily, which can be increased by 500 mg per week up to a maximum of 2000 mg taken once daily at night. If needed, the dose may be divided into 1000 mg taken twice daily, not exceeding 2000 mg per day.

Polycystic ovary syndrome may be treated with an initial dose of 500 mg daily in the morning for 1 week, followed by 500 mg twice daily for another week, and then 1.5-1.7 grams per day in 2-3 divided doses.

Elderly patients should start with conservative initial and maintenance dosing.

Child Dose: For children above 10 years old, the initial dose is usually 500 mg once or twice daily or 850 mg once daily, which may be increased gradually up to a maximum of 2000 mg per day in 2 or 3 divided doses at weekly intervals.

Renal Dose: In patients with renal impairment, Bigmet 850mg Tablet use is contraindicated if the estimated glomerular filtration rate (eGFR) is below 30 mL/min/1.73 m². It is not recommended to initiate treatment if eGFR is between 30-45 mL/min/1.73 m². For those at risk for renal impairment (e.g., elderly), eGFR should be monitored at least annually or more frequently. If eGFR falls below 45 mL/min/1.73 m² while taking Bigmet 850mg Tablet, the risks and benefits of continuing therapy should be evaluated. If eGFR falls below 30 mL/min/1.73 m² while taking Bigmet 850mg Tablets, the drug should be discontinued.

Always follow the advice of your doctor regarding the dosing and administration of Bigmet 850mg Tablet. The tablets should be taken with meals to reduce stomach or bowel side effects and swallowed with a full glass of water. The medicine should only be used under the supervision of a doctor.

Interaction

Bigmet 850mg Tablet can interact with several drugs, including anti-depressants (bupropion), glaucoma medications, certain antibiotics (cephalexin, ciprofloxacin), anti-acidity drugs (cimetidine), heart medications (digoxin), anti-HIV drugs (dolutegravir), and others. These interactions may lead to potential side effects or altered drug effectiveness.

In addition, Bigmet 850mg Tablet can interact with certain foods, specifically excessive alcohol consumption, which may increase the risk of a serious condition called Lactic Acidosis. Therefore, it is important to avoid consuming alcohol while taking this medication.

People with certain medical conditions, such as heart diseases (congestive heart failure and myocardial infarction), Vitamin B12 deficiency, and alcoholism, should avoid taking Bigmet 850mg Tablet as it may worsen their condition.

Always inform your healthcare provider about all the medications, supplements, and medical conditions you have to ensure safe and effective treatment with Bigmet 850mg Tablet.

Contraindications
  • If you are allergic to metformin or other ingredients of Bigmet 850mg Tablet.
  • You may have kidney failure or liver disease.
  • You may have several conditions such as dehydration, shock, and severe infection.
  • You may have an acid-base imbalance in the body or diabetes complications like diabetic ketoacidosis.
  • If you have conditions like heart failure, lung failure, shock, heart attack, etc.

Pregnancy & Lactation

Uncontrolled diabetes (gestational or permanent) during pregnancy is linked to an increased risk of congenital malformations and perinatal mortality. When a patient intends to become pregnant or is pregnant, it is suggested that diabetes be managed with insulin rather than metformin to keep blood glucose levels as close to normal as possible, reducing the risk of fetal abnormalities.

Metformin is secreted in human breast milk during breastfeeding. In breastfed newborns/infants, no harmful effects were identified. Breastfeeding is not recommended when using metformin because there is insufficient data. The benefits of nursing and the potential danger of detrimental effects on the kid should be considered while deciding whether or not to stop breastfeeding.

Precautions & Warnings

Some diabetic patients taking Bigmet 850mg Tablet may develop a serious condition called lactic acidosis, where there is an accumulation of too much lactic acid in the blood. Proper liver and kidney function are essential for eliminating excess lactic acid. Avoid taking metformin if you have kidney disease, as measured by a blood test. Regular blood tests for kidney function and vitamin B-12 levels are recommended as Bigmet 850mg Tablet may lower vitamin B-12 levels.

When used with insulin, Bigmet 850mg Tablet may significantly lower blood sugar levels, and the doctor may need to adjust the insulin dose accordingly. Regular monitoring and communication with your doctor are important to ensure safe and effective diabetes management.

Storage Conditions

Store Bigmet 850mg Tablets below 25°C at a cool and dry place. Protect it from direct sunlight and moisture. Keep it out of reach of children and pets.
