If needed, the dose may be divided into 1000 mg taken twice daily, not exceeding 2000 mg per day.

Polycystic ovary syndrome may be treated with an initial dose of 500 mg daily in the morning for 1 week, followed by 500 mg twice daily for another week, and then 1.5-1.7 grams per day in 2-3 divided doses.

Elderly patients should start with conservative initial and maintenance dosing.

Child Dose: For children above 10 years old, the initial dose is usually 500 mg once or twice daily or 850 mg once daily, which may be increased gradually up to a maximum of 2000 mg per day in 2 or 3 divided doses at weekly intervals.

Renal Dose: In patients with renal impairment, Bigmet 850mg Tablet use is contraindicated if the estimated glomerular filtration rate (eGFR) is below 30 mL/min/1.73 m². It is not recommended to initiate treatment if eGFR is between 30-45 mL/min/1.73 m². For those at risk for renal impairment (e.g., elderly), eGFR should be monitored at least annually or more frequently. If eGFR falls below 45 mL/min/1.73 m² while taking Bigmet 850mg Tablet, the risks and benefits of continuing therapy should be evaluated. If eGFR falls below 30 mL/min/1.73 m² while taking Bigmet 850mg Tablets, the drug should be discontinued.
