Apuldon is used for the symptomatic treatment of certain gastrointestinal conditions, including:

  • Dyspeptic symptoms: Abdominal fullness, bloating, upper abdominal discomfort, belching, flatulence, early satiety, nausea, vomiting, and heartburn associated with delayed gastric emptying or gastroesophageal reflux.
  • Non-ulcer dyspepsia: Helps relieve symptoms associated with functional indigestion.
  • Acute nausea and vomiting: May be used for nausea and vomiting associated with certain functional, organic, infectious, dietary, drug-related, radiotherapy-related, or migraine-related conditions.
  • Dopamine-agonist-induced nausea and vomiting: May be used in patients receiving dopamine agonists, including those with Parkinson's disease.
  • Radiological procedures: Domperidone may be used in selected circumstances to facilitate gastrointestinal transit during certain radiological studies.

Domperidone should generally be used for the shortest duration necessary. Do not use Apuldon for longer than 7 days unless specifically instructed by your doctor.

✔️ Drug Interactions

Domperidone may interact with several medicines.

CYP3A4 inhibitors, including certain azole antifungals and macrolide antibiotics, can increase domperidone concentrations and may increase the risk of cardiac adverse effects.

Domperidone should also not be combined with medicines that significantly prolong the QT interval where such combinations are contraindicated.

Other interactions include:

  • Anticholinergic medicines: May reduce the gastrointestinal effects of domperidone.
  • Antacids and antisecretory medicines: May reduce the absorption of orally administered domperidone when taken simultaneously.
  • Dopamine agonists: Domperidone may reduce their peripheral gastrointestinal adverse effects without significantly affecting their central therapeutic action.
✔️ Apuldon should not be used in patients with:
  • Known hypersensitivity to domperidone or any component of the formulation.
  • Gastrointestinal bleeding, mechanical obstruction, or perforation where increased gastrointestinal motility could be harmful.
  • Prolactin-secreting pituitary tumour (prolactinoma).
  • Certain significant heart rhythm disorders or conditions associated with QT prolongation.
  • Moderate or severe hepatic impairment.
  • Concomitant treatment with medicines that significantly prolong the QT interval or strongly inhibit CYP3A4, where contraindicated.
✔️ Dosage and Administration

Apuldon should generally be taken 15–30 minutes before meals. Always follow the dose and duration prescribed by your doctor.

Adults:

The usual oral dose is 10–20 mg (10–20 ml of the 5 mg/5 ml suspension) every 6–8 hours, depending on the indication.

The maximum daily dose should not exceed the amount recommended by the prescribing physician.

For dyspeptic symptoms:

  • Adults: 10–20 mg (10–20 ml suspension) every 6–8 hours.

For acute nausea and vomiting:

  • Adults: 20 mg (20 ml suspension) every 6–8 hours, when specifically prescribed.

Children:

Pediatric use and dosing should be determined by a physician based on the child's body weight, age, and clinical condition. Domperidone is not routinely recommended for young children, and it should not be given to infants without specialist medical supervision.

Elderly:
Older adults may be more susceptible to cardiac adverse effects. The lowest effective dose should be used for the shortest appropriate duration under medical supervision.

Liver impairment:
Because domperidone is extensively metabolized in the liver, it should generally be avoided or used with particular caution in patients with significant hepatic impairment.

Kidney impairment:
In severe renal impairment, repeated dosing may require a reduction in dose and/or dosing frequency. Patients receiving prolonged treatment should be monitored regularly.

  • Use Apuldon cautiously in patients with liver impairment.
  • Patients with significant kidney impairment may require a reduced dose or less frequent administration during repeated treatment.
  • Domperidone should be used cautiously in patients with a history of heart rhythm problems, electrolyte abnormalities, or other risk factors for QT prolongation.
  • Tell your doctor about all medicines you are taking, particularly antibiotics, antifungal medicines, heart medicines, and other medicines that may affect cardiac rhythm.
  • Stop taking the medicine and seek medical advice if you develop palpitations, fainting, or severe dizziness.
  • Do not exceed the prescribed dose or duration of treatment.
  • If nausea or vomiting persists, recurs, or is accompanied by severe abdominal pain, fever, or persistent watery diarrhoea, consult a doctor.
