✔️ Side Effects of Umactin 50

• Common Side Effects
• Gastrointestinal: Nausea, vomiting, diarrhea, anorexia, abdominal discomfort
• CNS: Drowsiness, dizziness, headache, vertigo
• Dermatologic: Rash, pruritus, urticaria
• Urine discoloration: Brown or dark yellow urine (benign)

Serious and Rare Side Effects

Potentially Fatal:

• Peripheral neuropathy
• Hepatotoxicity
• Pulmonary fibrosis, interstitial pneumonitis
• Stevens-Johnson syndrome, anaphylaxis
• Hematologic disorders: Aplastic anemia, hemolytic anemia (especially in G6PD-deficient patients), agranulocytosis

✔️ Indication of Umactin 50

• Acute uncomplicated urinary tract infections (cystitis)
• Prophylaxis of recurrent uncomplicated UTIs

✔️ Patient Guidance

• Take with food to reduce nausea.
• Complete the full course even if symptoms improve early.
• Do not use if pregnant at term or if breastfeeding a newborn.
• Urine discoloration is normal and harmless.
• Report any numbness, breathing issues, or rash immediately.

✔️ Dosage & Administration of Umactin 50

Adults

Acute UTI:

• Standard tablets: 50–100 mg orally 4 times daily for 7 days or 3 days after sterile urine is obtained
• Sustained-release tablets (SR): 100 mg orally twice daily for 7 days or 3 days after sterile urine
• Prophylaxis: 50–100 mg orally at bedtime, up to 12 months

Children (>1 month)

• Treatment: 5–7 mg/kg/day orally, divided every 6 hours for 7 days
• Prophylaxis: 1–2 mg/kg orally at bedtime or in 2 divided doses

Age ≥12 years (SR): 100 mg orally twice daily for 7 days or 3 days post-sterility

Tablets should be taken with food to improve absorption and reduce gastrointestinal upset.

Renal Dose Adjustment

• CrCl <60 mL/min: Avoid use due to poor urinary concentration and increased toxicity risk

Monitor renal function, especially in elderly patients

✔️ Drug Interactions

• Probenecid and sulfinpyrazone: Decrease renal excretion of nitrofurantoin → risk of toxicity
• Magnesium trisilicate: Reduces absorption
• Quinolone antibiotics: Antagonistic effect
• Urinary alkalinizers or carbonic anhydrase inhibitors: Reduce efficacy

✔️ Contraindications

• Severe renal impairment (CrCl <60 mL/min)
• Known hypersensitivity to nitrofurantoin or other nitrofurans
• G6PD deficiency
• Infants <3 months of age
• Pregnancy at term (38–42 weeks), during labor and delivery

✔️ Precautions

• Elderly and those with renal, hepatic, pulmonary, or neurological disorders
• Monitor hepatic and pulmonary function during prolonged therapy
• Risk of peripheral neuropathy in patients with renal impairment, diabetes, anemia, or vitamin B deficiency
• Discontinue immediately if neuropathy or lung symptoms appear
• Avoid during breastfeeding if the infant is <1 month old

✔️ Storage:

Store in a cool and dry place away from light and heat. Keep all medicines out of the reach of children.
