Mesalamine primarily acts locally on the intestinal mucosa rather than systemically. In inflammatory bowel disease, the production of arachidonic acid metabolites such as prostaglandins and leukotrienes is increased. Mesalamine reduces intestinal inflammation by inhibiting cyclooxygenase activity and suppressing prostaglandin synthesis in the colon.
