Azithromycin, an acid-stable macrolide, is well absorbed orally and accumulates in phagocytes, concentrating at infection sites. It inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. It has a long half-life (~68 hours), allowing once-daily dosing and short-course therapy. Primarily excreted in bile; ~6% is excreted in urine unchanged.
Adults: 500 mg once daily for 3 days or as 500 mg on day 1, followed by 250 mg once daily for the next 4 days.
Chlamydia infections: 1 g single dose or alternative 3-day regimen (500 mg, then 250 mg for 2 days).
Children (based on weight):
Typhoid fever: 500 mg daily for 7–10 days
Pregnancy (Category B): Use only if necessary; no fetal harm seen in animals
Lactation: Unknown if excreted in breast milk; use with caution
Renal Impairment: No adjustment needed in mild cases; caution in severe cases
