✅ Description:
Cinalid 500 is a broad-spectrum macrolide antibiotic used to treat a wide range of bacterial infections affecting the respiratory tract, skin, ears, eyes, and certain sexually transmitted infections. It is also effective in typhoid fever and community-acquired pneumonia.
Azithromycin binds to the 50S ribosomal subunit of bacteria, inhibiting protein synthesis by blocking transpeptidation. This action stops bacterial growth and replication.

✅ Adults:
Most infections: 500 mg once daily for 3 days
Chlamydia / Cervicitis / Urethritis / Chancroid: 1 g single dose
Gonorrhea: 2 g single dose
MAC prophylaxis: 1.2 g once weekly
MAC treatment: 500 mg daily with other antimicrobials
✅ Children:
Pharyngitis: 12 mg/kg/day for 5 days
Otitis media: 10 mg/kg/day × 1 day, then 5 mg/kg/day for 4 days
Pneumonia: 10 mg/kg × 1 day, then 5 mg/kg/day for 4 days
Max dose: 500 mg/day
✅ Administration:
Take 1 hour before or 2 hours after meals
Do not take with antacids (separate by 2 hours)
✔️ Interaction
May increase levels of: digoxin, phenytoin, ciclosporin, terfenadine
Avoid antacids containing Mg/Al (reduce absorption)
Ergot derivatives: Risk of ergotism
Serious interaction with pimozide (risk of arrhythmia)
✔️ Pregnancy & Lactation:
- Available human data do not indicate increased risk of birth defects or adverse pregnancy outcomes.
- Use with caution during breastfeeding (drug excretion in milk not fully established)
✔️ Precautions:
- Use with caution in patients with hepatic or renal impairment, prolonged QT interval, low potassium or magnesium, or existing heart arrhythmias
- Monitor LFTs and CBC if used long-term
- Not to be co-administered with pimozide (risk of cardiotoxicity)
