Tenoloc 50 Tablet

✔️ Dosage and Administration

Hypertension

Adults

• Initial dose: 25–50 mg once daily.
• Dose may be increased up to 100 mg daily depending on response.

Elderly Patients

• Initial dose: 25 mg once daily.
• Dose may be adjusted gradually as needed.

Angina Pectoris

Adults

• Initial dose: 50 mg once daily.
• After one week, the dose may be increased to 100 mg daily.
• Some patients may require up to 200 mg daily.

Elderly Patients

• Initial dose: 25 mg daily.
• May be increased gradually according to clinical response.

Post-Myocardial Infarction (Heart Attack)

• Secondary prevention dose: 100 mg daily, either as a single dose or divided into two doses every 12 hours.
• Treatment is generally continued for 6–9 days following myocardial infarction, according to medical advice.

Administration

• May be taken with or without food.
• Swallow the tablet whole.
• Do not crush, chew, or break the tablet.
• Take it at the same time every day for consistent therapeutic effects.

✔️ Side Effects

• Fatigue
• Slow heart rate (bradycardia)
• Cold hands and feet
• Nausea
• Diarrhea
• Dizziness
• Low blood pressure
• Drowsiness
• Lightheadedness
• Vertigo
• Shortness of breath
• Depression
• Abnormal dreams
• Leg pain
• Weakness
• Visual disturbances
• Skin rashes

✔️ Excessive doses may result in:

• Severe bradycardia
• Marked hypotension
• Heart block
• Acute heart failure
• Breathing difficulties

✔️ Tenoloc 50 should not be used in patients with:

• Known hypersensitivity to atenolol
• Sinus bradycardia
• Significant sinus node dysfunction
• Second- or third-degree heart block
• Cardiogenic shock
• Uncompensated heart failure
• Severe bronchospastic disease
• Significant peripheral vascular disease

Use during pregnancy should be considered only when clearly necessary and under medical supervision.

✔️ Renal Impairment

Dose adjustment may be required in patients with reduced kidney function:

• Creatinine clearance (CrCl) 15–35 mL/min: 50 mg once daily.
• CrCl below 15 mL/min: 25–50 mg on alternate days.
• Dialysis patients: 25–50 mg after each dialysis session.

✔️ Pregnancy and Breastfeeding

Pregnancy

• Atenolol should be used during pregnancy only if the expected benefits outweigh potential risks.
• Close monitoring may be necessary.

Breastfeeding

• Atenolol passes into breast milk.
• Infants exposed through breastfeeding may be at risk of hypoglycemia and bradycardia.
• Use cautiously during lactation.

✔️ Atenolol may interact with several medications, including:

• Reserpine, which may increase the risk of excessive hypotension and bradycardia.
• Calcium channel blockers (e.g., verapamil and diltiazem), which may increase the risk of heart block and severe slowing of the heart rate.
• Hydralazine and methyldopa, which may enhance blood pressure-lowering effects.
• NSAIDs (e.g., indomethacin), which may reduce antihypertensive effectiveness.
• Amiodarone, disopyramide, and digitalis glycosides, which may further slow the heart rate.
• Clonidine, where improper withdrawal may increase the risk of rebound hypertension.
