✔️ Side Effects

• Sinus congestion or sinusitis
• Back pain
• Diarrhea
• Intermittent claudication (leg pain due to reduced blood flow)
• Skin ulcers

✔️ Dosage and Administration

Treatment should be individualized according to patient needs and response.

Hypertension

• Recommended starting dose: 40 mg once daily
• Effective dose range: 20–80 mg once daily
• Most blood pressure reduction is seen within 2 weeks.
• Maximum therapeutic effect is generally achieved after approximately 4 weeks.

If additional blood pressure control is required after reaching 80 mg daily, combination therapy with other antihypertensive medications may be considered.

Special Populations

Elderly Patients

• No routine dosage adjustment is required.
• Initial therapy is generally not recommended in patients aged 75 years or older without careful medical assessment.

Renal Impairment

• No initial dosage adjustment is usually necessary.
• Patients undergoing hemodialysis may be at increased risk of orthostatic hypotension and should be monitored closely.

Hepatic Impairment

• Use cautiously in patients with liver dysfunction or biliary obstruction.
• Dose increases should be gradual with careful monitoring.

Pediatric Patients

• Safety and effectiveness have not been established.

• May be taken with or without food.
• Take the medication at the same time each day for optimal blood pressure control.
• Continue treatment even if symptoms are not noticeable, as hypertension often causes no symptoms.

✔️ Use with caution in patients with:

• Liver impairment
• Kidney impairment
• Volume depletion
• Low blood pressure

Important precautions include:

• Avoid use during pregnancy due to potential harm to the fetus.
• Monitor kidney function and blood pressure regularly.
• Avoid combining an ACE inhibitor with an ARB unless specifically directed by a specialist.
• Patients receiving dialysis should be monitored for orthostatic hypotension.

✔️ Drug Interaction

NSAIDs

Examples include ibuprofen, naproxen, and diclofenac.

Potential effects:

• Reduced antihypertensive efficacy
• Increased risk of kidney impairment

Aliskiren

• Concurrent use with aliskiren should be avoided in patients with diabetes due to increased risks of:
  • Hyperkalemia
  • Hypotension
  • Kidney dysfunction

✔️ Pregnancy and Breastfeeding

Pregnancy

• Pregnancy Category C during the first trimester.
• Pregnancy Category D during the second and third trimesters.
• Telmisartan can cause serious fetal injury or death and should be discontinued immediately if pregnancy occurs.

Breastfeeding

• Because of the potential risk to nursing infants, either breastfeeding or the medication should be discontinued based on clinical necessity.

✔️ Telisa should not be used in patients with:

• Known hypersensitivity to telmisartan
• Allergy to any component of the formulation

• Store below 30°C.
• Protect from light and excessive humidity.
• Keep out of reach of children.
