Telmisartan is a non-peptide angiotensin II receptor antagonist. Angiotensin II is a hormone that causes blood vessels to constrict and stimulates aldosterone release, leading to increased blood pressure and sodium retention.

Telmisartan works by selectively blocking the AT1 receptors to which angiotensin II binds. This results in:

  • Relaxation of blood vessels
  • Reduced blood pressure
  • Decreased aldosterone secretion
  • Reduced sodium and water retention
  • Lower workload on the heart

Telmisartan has a very high affinity for AT1 receptors and does not significantly affect AT2 receptors. Unlike ACE inhibitors, it does not interfere with bradykinin metabolism and therefore is less likely to cause a persistent dry cough.

Hypertension

Elderly Patients

Renal Impairment

  • No initial dosage adjustment is usually necessary.
  • Patients undergoing hemodialysis may be at increased risk of orthostatic hypotension and should be monitored closely.

Hepatic Impairment

  • Use cautiously in patients with liver dysfunction or biliary obstruction.
  • Dose increases should be gradual with careful monitoring.

Pediatric Patients

  • Safety and effectiveness have not been established.

 

  • May be taken with or without food.
  • Take the medication at the same time each day for optimal blood pressure control.
  • Continue treatment even if symptoms are not noticeable, as hypertension often causes no symptoms.

Aliskiren
