✅ Description:
R-Pil is prescribed for:
- Hypertension (High Blood Pressure): as single-drug therapy or with other antihypertensives
- Congestive Heart Failure (CHF): including post-myocardial infarction
- Prevention of cardiovascular events: in patients at high risk (history of heart attack, stroke, peripheral vascular disease, or diabetes with risk factors)
- Diabetic or non-diabetic nephropathy: to protect the kidneys

✔️ Dosage & Administration of R Pil 5 Tablet
Adults:
Hypertension: Start 1.25–2.5 mg once daily; adjust every 2 weeks; maintenance 2.5–20 mg daily (single or divided dose)
Post-myocardial infarction CHF: Start 2.5 mg twice daily (reduce to 1.25 mg if hypotension occurs); target 5 mg twice daily
Prevention of cardiovascular events: 2.5 mg once daily (week 1), 5 mg once daily (weeks 2–4); maintenance up to 10 mg daily as tolerated
Renal Impairment:
Start lower: 1.25 mg daily; adjust according to BP response and tolerance
Max dose in renal impairment: 5 mg daily
Administration:
Swallow the tablet with water; do not chew or crush
Can be taken before, during, or after meals

Dizziness, headache, fatigue, asthenia
Less common: cough, nausea, vomiting, diarrhea, rash, urticaria
Rare but serious: angioneurotic edema, anaphylaxis, hyperkalemia
✔️ Do not use R-Pil if you:
- Are allergic to ramipril or other ACE inhibitors
- Have history of angioedema
- Are taking sacubitril/valsartan, aliskiren (in diabetes or renal impairment), or AIIRAs in diabetic nephropathy
- Have bilateral renal artery stenosis
- Are pregnant or breastfeeding
- Are hypotensive or hemodynamically unstable
✔️ Drug Interactions
- Diuretics: may cause severe hypotension or hyperkalemia
- Lithium: may increase lithium levels
- NSAIDs: may reduce the antihypertensive effect and worsen kidney function
- BP lowering may affect driving or operating machinery, worsened by alcohol
✔️ Use with caution in:
- Renal impairment, hyperkalemia, hypotension, liver impairment
- Elderly: start with 1.25 mg daily
- Patients previously on diuretics: consider stopping or reducing diuretics 2–3 days before starting R-Pil
- Avoid sudden discontinuation
✔️ Storage:
Store below 30°C, away from light and moisture
