Bisoprolol is a β1-selective (cardioselective) adrenergic receptor blocker. By blocking sympathetic stimulation of the heart:

Common:
- Fatigue, dizziness, headache
- Nausea, vomiting, diarrhea, or constipation
- Cold hands and feet
Less common but significant:
- Slow heart rate (bradycardia)

- Digitalis glycosides (increased risk of bradycardia)
- Calcium channel blockers (verapamil, diltiazem) and antiarrhythmics (risk of AV block, heart failure)
- Clonidine: avoid abrupt withdrawal if taken together
- Insulin/antidiabetic drugs: may mask hypoglycemia symptoms

- Pregnancy & Lactation: Not recommended. Should only be used if benefits outweigh risks.
- Respiratory conditions: Contraindicated in severe asthma or chronic lung disease.
- Cardiac conditions: Avoid in patients with severe bradycardia, hypotension, cardiogenic shock, or advanced heart failure.
- Other contraindications: Metabolic acidosis, untreated phaeochromocytoma, severe peripheral circulatory disorders.
