Olsart is indicated for managing hypertension—either as monotherapy or in combination with other antihypertensive agents.
Olmesartan selectively blocks the binding of angiotensin II to AT1 receptors, thereby preventing its vasoconstrictive and aldosterone-secreting effects. This results in blood vessel relaxation, reduced blood pressure, and improved cardiovascular outcomes, without affecting bradykinin levels or interfering with ACE activity.
Generally well tolerated. Most common side effect: dizziness (3%). Other effects—similar to a placebo—include:
Usual starting dose: 20 mg once daily
If needed: Increase to 40 mg after 2 weeks
Max dose: 40 mg/day (no added benefit above this)
May be taken with or without food
No initial dose adjustment for elderly or patients with moderate renal/hepatic impairment
Pediatric use: Not established
Start under medical supervision, especially in volume-depleted patients.
Pregnancy: Contraindicated
Lactation: Discontinue drug or breastfeeding based on risk-benefit assessment
Monitor renal function, especially in CHF or renal-compromised patients
Use caution in patients at risk of renal impairment or hypovolemia
Avoid during pregnancy (especially 2nd/3rd trimester) due to the risk of fetal harm
