Indications
Bizoran 5/40 contains two medications that help control high blood pressure. Lowering your blood pressure lowers your chances of having a heart attack or stroke in the future. Bizoran 5/40 can be taken either on an empty stomach or with food. However, it is preferable to take it on a daily basis at the time prescribed by your doctor.
Pharmacology
Amlodipine is a dihydropyridine calcium channel blocker that can prevent calcium ions from crossing the blood-brain barrier and entering the vascular smooth muscle and myocardium. Amlodipine has a greater effect on vascular smooth muscle cells than on cardiomyocytes. Amlodipine is a peripheral arterial vasodilator that works directly on vascular smooth muscle to reduce peripheral vascular resistance and blood pressure.
Angiotensin II is created through a reaction catalyzed by the angiotensin-converting enzyme (ACE). It is a potent vasoconstrictor as well as the primary vasoactive hormone of the renin-angiotensin system. It is also a part of the pathophysiology to have high blood pressure. It also causes the adrenal cortex to produce aldosterone.
Olmesartan medoxomil inhibits angiotensin II vasoconstriction and aldosterone secretion by selectively blocking angiotensin II binding to AT1 receptors found in many tissues (such as vascular smooth muscle, and adrenal glands). Olmesartan Medoxomil is a reversible competitive inhibitor of the AT1 receptor, according to in vitro binding studies. Olmesartan medoxomil has no effect on ACE (kinase II, an enzyme that converts angiotensin I into angiotensin II and breaks down bradykinin).
Dosage & Administration
For patients on Bizoran, use individually titrated components. As needed, this combination may also be given with higher doses of Amlodipine, Olmesartan Medoxomil, or both.
Begin with 5/20 mg once daily for 1 to 2 weeks, then gradually increase to a maximum of 10/40 mg once daily. Due to decreased clearance of Amlodipine in elderly patients, the recommended starting dose of Amlodipine in patients 75 years old is 2.5 mg. Because the combination's lowest dose is 5/20 mg, initial therapy with this combination drug is not recommended in patients over the age of 75.
Interaction
Nonsteroidal anti-inflammatory drugs (NSAIDs) can reduce the antihypertensive effects of angiotensin II receptor antagonists (including olmesartan medoxomil) (including selective COX2 inhibitors). Patients on combination therapy and other medications that affect RAS should have their blood pressure, kidney function, and electrolytes closely monitored.
Contraindications
Diabetes patients cannot be combined with Aliskiren.
Side-Effects
Pregnancy & Lactation
Pregnancy category D. The combination of amlodipine and olmesartan medoxomil should not be used in the second and third trimesters as it can cause fetal death. When pregnancy is detected, this combination should be stopped as soon as possible. It is not clear whether olmesartan and amlodipine are excreted in human milk. Due to the possible adverse effects of breastfeeding babies, the decision should be based on the importance of the drug to the mother whether to stop breastfeeding or to stop taking the drug.
Precautions & Warnings
The combination of amlodipine and olmesartan medoxomil should be used with caution because of the risk of volume hypotension or salt depletion. Vasodilation in patients with severe aortic stenosis. Increased frequency, duration, or severity of angina or acute Ml in patients with severe obstructive coronary artery disease.
Storage Conditions
Do not store above 30 ° C. Keep away from light and out of the reach of children.
