Severe Hypothyroidism: Begin with 12.5–25 mcg/day; increase by 25 mcg/day every 2–4 weeks as needed.

Pediatric Dosing

Neonates & Infants (1–12 months): 6–15 mcg/kg/day.

Children (1–12 years): 4–6 mcg/kg/day.

Adolescents (>12 years): 2–3 mcg/kg/day or up to 150 mcg/day.

Dosing should be individualized based on body weight, age, and disease severity.
