✔️ Overdose Management

There is no specific antidote for atorvastatin overdose. Treatment is supportive and symptom-based, with monitoring of:

• Liver function tests
• Creatine kinase (CK) levels

Hemodialysis is unlikely to be beneficial.

✔️ Dosage and Administration

Adults

Primary Hypercholesterolemia and Combined Hyperlipidemia

• Usual starting dose: 10 mg once daily.
• Dose may be increased every 4 weeks if needed.
• Maximum dose: 80 mg daily.

Familial Hypercholesterolemia

• Start with 10 mg daily.
• Gradually increase to 40 mg daily, and if necessary up to 80 mg daily.

Prevention of Cardiovascular Events

• Initial dose: 10 mg once daily, adjusted according to response.

Pediatric Patients (10–17 years)

• Recommended starting dose: 10 mg daily.
• Maximum recommended dose: 20 mg daily.

Patients should follow a cholesterol-lowering diet before starting treatment and continue it throughout therapy. The medication can be taken with or without food, at any time of the day.

• Elderly: Higher blood concentrations may occur, often leading to greater LDL reduction.
• Renal impairment: No dose adjustment is generally required.
• Liver impairment: Drug levels may be significantly increased; careful monitoring is necessary.
• Hemodialysis patients: Dialysis does not effectively remove atorvastatin because it is highly protein-bound.

✔️ Drug Interactions

Certain medications can increase the risk of muscle-related side effects, including:

• Cyclosporine
• Fibrates
• Niacin
• Erythromycin
• Azole antifungal agents

Other interactions include:

• Antacids: May reduce atorvastatin blood levels but generally do not affect LDL reduction.
• Colestipol: Lowers atorvastatin concentration but may enhance cholesterol-lowering effects.
• Digoxin: Digoxin levels may increase; monitoring is advised.
• Oral contraceptives: May increase hormone exposure.
• Warfarin: Usually has no significant effect on clotting time.

✔️ Atorvastatin should not be used in:

• Individuals allergic to atorvastatin or any component of the formulation.
• Patients with active liver disease or unexplained persistent elevations of liver enzymes.
• Those with a history of severe adverse reactions to statins.

✔️ Pregnancy & Lactation

• Pregnancy: Contraindicated because it may interfere with fetal development and cholesterol synthesis.
• Breastfeeding: Should not be used, as it may pass into breast milk and potentially harm the infant.

✔️ Precautions and Warnings

• Liver function tests should be performed before starting therapy and monitored periodically.
• Use cautiously in people with a history of liver disease or significant alcohol consumption.
• Treatment should be discontinued if severe muscle injury (myopathy) or markedly elevated creatine kinase (CK) levels occur.

✔️ Storage:

Store in a cool, dry place away from heat and direct light. Keep out of reach of children.
