✅ Description:
- Severe hypertriglyceridemia with or without low HDL cholesterol.
- Mixed hyperlipidemia when statins are contraindicated or not tolerated.
Pharmacology:
Ciprofibrate lowers LDL and VLDL, reducing triglyceride and cholesterol levels while increasing HDL. It is effective in treating hyperlipidemia and may reduce coronary heart disease events.
Dosage & Administration:
- Adults: 100 mg per day (do not exceed).
- Renal impairment: Reduce to one tablet every other day for moderate impairment. Avoid use in severe renal impairment.
- Hepatic insufficiency: Use cautiously; discontinue if liver enzymes increase more than 3 times the upper limit of normal.
- Elderly (>70 years): Same dose as adults but with precautions.
- Pediatrics: Not recommended.
Interactions:
- Other fibrates & statins: Increased risk of myopathy and rhabdomyolysis.
- Oral anticoagulants: May require dose adjustment of anticoagulants.
Contraindications:
- Hypersensitivity, severe hepatic/renal impairment, pregnancy, lactation, and concurrent use with another fibrate.
Side Effects:
Common effects include headache, dizziness, nausea, vomiting, diarrhea, rash, alopecia, and muscle pain.
Pregnancy & Lactation:
- Contraindicated in pregnancy and lactation due to lack of safety data.
Precautions:
- Monitor muscle pain for myopathy.
- Periodic liver function tests are recommended.
