✔️ Emep MUPS is indicated for:

• Gastro-esophageal Reflux Disease (GERD)
• Risk reduction of NSAID-associated gastric ulcers
• H. pylori eradication (as part of triple therapy)
• Zollinger-Ellison syndrome and idiopathic hypersecretion

Use as directed by a registered physician.

✔️ Pharmacology

Esomeprazole, a proton pump inhibitor, blocks the hydrogen-potassium ATPase enzyme system (proton pump) in gastric parietal cells, effectively reducing gastric acid secretion. It is used for the short-term treatment of gastric and duodenal ulcers and for preventing NSAID-induced gastric ulcers.

✔️ Dosage & Administration

Erosive Esophagitis

• Adults (≥18 years): 40 mg once daily for 4 weeks.
• Children (12-18 years): 40 mg once daily for 4 weeks.

Maintenance of Healing (Erosive Esophagitis)

• Adults (≥18 years): 20 mg once daily.
• Children (12-18 years): 20 mg once daily.

Risk Reduction of NSAID-associated Gastric Ulcers

• Adults (≥18 years): 20 mg once daily for 4-8 weeks.

H. pylori Eradication (with Amoxicillin and Clarithromycin)

• Adults (≥18 years): 20 mg twice daily for 7 days.
• Children (12-18 years): 20 mg twice daily for 7 days.

Zollinger-Ellison Syndrome & Idiopathic Hypersecretion

• Adults (≥18 years): 40-80 mg twice daily.

Children (1-11 years)

• Erosive esophagitis:
  • <20 kg: 10 mg once daily for 8 weeks.
  • ≥20 kg: 10-20 mg once daily for 8 weeks.
• Maintenance: 10 mg once daily.

Not approved for children below 1 year.

✔️ Administration

• Swallow the tablets whole with liquid.
• Do not chew or crush the tablets.
• Alternatively, disperse the tablet in half a glass of non-carbonated water (not mineral water). Stir until disintegrated, drink immediately, and rinse the glass with more water to ensure full dose intake.

✔️ Interactions

• Metabolism: Primarily metabolized by CYP2C19 and CYP3A4.
• CYP Interactions: No clinically significant interactions with CYP1A2, 2A6, 2C9, 2D6, or 3A4.
• Drug Absorption: This may affect drugs dependent on gastric pH for absorption (e.g., ketoconazole, iron salts, digoxin).
• Combination Therapy: Increases plasma levels of Emep MUPS and 14-hydroxy clarithromycin when co-administered with clarithromycin and amoxicillin.

✔️ Side Effects

Common side effects include:

• Headache
• Diarrhea
• Nausea
• Flatulence
• Abdominal pain
• Constipation
• Dry mouth

No significant differences in side effects were observed between short-term and maintenance treatment (up to 12 months).

✔️ Use During Pregnancy & Lactation

• Use with caution during pregnancy.
• It is unclear if Esomeprazole or its metabolites are excreted in human breast milk.

✔️ Contraindications

• Known hypersensitivity to Esomeprazole or any component of the formulation.

✔️ Precautions & Warnings

• Use cautiously in patients with severe liver or renal impairment.
• Prolonged use (>1 year) may increase the risk of fractures (hip, wrist, spine).

• Keep out of reach of children.
