Dasacent is a medication primarily used for treating Philadelphia chromosome-positive (Ph+) chronic myeloid leukemia (CML) and acute lymphoblastic leukemia (ALL) in both adult and pediatric patients.
- Adults:
- Newly diagnosed Ph+ CML in chronic phase.
- Chronic, accelerated, or blast phase Ph+ CML with resistance or intolerance to prior therapy.

- Mechanism of Action: Dasatinib inhibits several kinases including BCR-ABL, SRC family, c-KIT, EPHA2, and PDGFRβ. This inhibition helps overcome Imatinib resistance and targets various signaling pathways involved in leukemia progression.
- Absorption: Peak plasma concentrations are reached between 0.5 to 6 hours after oral administration.
- Metabolism: Primarily metabolized by CYP3A4, with minor contributions from other enzymes like FMO-3 and UGT.
- Excretion: Mostly excreted in the feces, with only 4% excreted in urine.

- Adult Dosage:
- Chronic Phase CML: 100 mg orally once daily.
- Accelerated phase, blast phase CML, or Ph+ ALL: 140 mg orally once daily.
- Pediatric Dosage:
- Based on weight (starting from 40 mg/day for children weighing 10-20 kg, to 100 mg/day for those weighing 45 kg or more).

- CYP3A4 Inhibitors: Can increase Dasacent concentrations and may raise the risk of toxicity.
- CYP3A4 Inducers: Can decrease Dasacent concentrations and reduce efficacy.
- Gastric Acid Reducing Agents: May reduce Dasacent's effectiveness; avoid use with H2 antagonists or proton pump inhibitors.

- Pregnancy: Dasatinib can cause fetal harm and should not be used during pregnancy unless necessary.
- Breastfeeding: Not recommended due to excretion in breast milk.
