Pharmacology

Tranexamic acid operates through competitive inhibition of the activation of plasminogen. Plasminogen is activated to plasmin, an enzyme responsible for breaking down fibrin clots, fibrinogen, and other plasma proteins. The inhibition occurs through binding to the kringle domain of plasminogen.

The primary actions include:

  1. Inhibition of Plasmin Formation: Tranexamic acid reduces the conversion of plasminogen to plasmin, hindering the formation of this fibrinolytic enzyme.
  2. Direct Inhibition of Plasmin Activity: Tranexamic acid also directly inhibits the activity of plasmin, although higher doses are necessary for this effect compared to the reduction in plasmin formation.

By interfering with the fibrinolytic process, tranexamic acid helps maintain hemostasis by preventing excessive breakdown of blood clots and fibrin components.

Pharmacokinetics:

  • Absorption: Tranexamic acid is rapidly absorbed from the gastrointestinal tract.
  • Peak Serum Levels: Maximum serum levels are achieved within 2-3 hours after administration.
  • Excretion (Oral Administration): Approximately 40% of the administered dose is excreted in the urine during the first 24 hours.
  • Excretion (Intravenous Administration): Following intravenous administration, 45% of the dose is excreted in the urine during the first day.
