Bionex contains Tranexamic Acid, a potent antifibrinolytic agent that inhibits the activation of plasminogen to plasmin — thereby preventing the breakdown of fibrin clots. It is around 10 times more active than conventional hemostatics.
It is rapidly absorbed orally, with effects visible within 15–30 minutes. The drug is mainly excreted by the kidneys and has a prolonged duration of action, allowing lower doses at longer intervals.
At therapeutic doses, Tranexamic Acid does not affect normal coagulation or cause thrombosis even with prolonged use.
Generally well-tolerated. Occasionally: fatigue, nasal congestion, mild skin rash, itching, nausea, or diarrhea. Rarely, postural hypotension or gastric irritation may occur.
Adults:
Usual dose: 500–1000 mg three times daily.
For prophylaxis: 0.5–1 g daily (or 500 mg IV/IM).
For hemorrhagic conditions: 1–3 g/day in divided doses.
In severe or urgent cases: Start with 500 mg slow IV injection, then continue oral therapy as needed.
Children:
Prophylaxis: 5–10 mg/kg/day orally in divided doses.
Therapeutic: 10–20 mg/kg/day orally; or 10 mg/kg slow IV injection.
Ampoules may be diluted and given orally for infants if preferred.
Elderly:
No dose adjustment is required unless renal impairment is present.
Incompatible with penicillin solutions.
Thrombolytics (e.g., Streptokinase, Urokinase) antagonize its antifibrinolytic effect.
Estrogen-containing contraceptives may increase the risk of thrombosis.
Avoid mixing directly with whole blood during transfusion.
Use is not recommended during pregnancy as placental transfer and fetal safety are not fully established.
Tranexamic Acid is excreted in small amounts in breast milk, but antifibrinolytic effects in infants are unlikely.
Store below 30°C, in a dry place away from light. Keep out of the reach of children.
