✔️ Dosage & Administration

Adults & Adolescents (≥15 years): 10 mg once daily.

Children 6–14 years: 5 mg once daily.

Children 2–5 years: 4 mg once daily (chewable tablet).

Children 6 months–5 years: 4 mg oral granules once daily (directly or mixed with cold water/soft food).

Elderly, renal impairment, mild-moderate hepatic impairment: No dose adjustment needed.

✔️ Side Effects

Common: Headache, diarrhea, fever, GI upset, nausea, URI, skin reactions.

Uncommon: Anxiety, dizziness, abnormal behavior, arthralgia, irritability, sleep disturbance.

Rare/Severe: Angioedema, hallucinations, hepatic dysfunction, pulmonary eosinophilia, suicidal ideation.

✔️ Interaction

Generally safe with commonly used asthma therapies and drugs such as theophylline, steroids, digoxin, warfarin, and oral contraceptives.

Phenobarbital & rifampin: Reduce plasma levels (~40%) due to hepatic enzyme induction → monitor clinically.

No major interactions reported with thyroid hormones, NSAIDs, benzodiazepines, decongestants, etc.

✔️ Contraindications:

Hypersensitivity to Montelukast or any component of the formulation.

✔️ Precautions & Warnings

Not for acute asthma attacks or status asthmaticus → patients must carry a rescue inhaler.

Can be continued during asthma exacerbations but should not replace corticosteroids abruptly.

Not to be used as sole therapy for exercise-induced bronchospasm.

Patients with aspirin-sensitive asthma must still avoid aspirin/NSAIDs.

Caution in pregnancy & lactation—use only if clearly needed.
