✔️ Pharmacology

Pregabalin is a derivative of gamma-aminobutyric acid (GABA), an inhibitory neurotransmitter. However, it does not directly interact with GABA receptors (GABAA, GABAB) or benzodiazepine receptors. Instead, Pregabalin binds with high affinity to the alpha2-delta site, an auxiliary subunit of voltage-gated calcium channels in the central nervous system. While its exact mechanism of action remains unclear, studies suggest that this binding contributes to its pain-relieving and anti-seizure properties.

✔️ Side Effects: Prelin 50

• Dizziness
• Drowsiness
• Dry mouth
• Swelling (edema)
• Blurred vision
• Weight gain
• Cognitive issues (difficulty concentrating or focusing)

✔️ Symptoms of overdose include

• Altered consciousness
• Anxiety or depression
• Confusion, agitation, or restlessness
• Seizures and heart block in some cases
• There is no specific antidote for Pregabalin overdose. Supportive measures include airway management, gastric lavage, and vital sign monitoring.

✔️ Neuropathic Pain Associated with Diabetic Peripheral Neuropathy (DPN) in Adults

• The maximum recommended dosage is 100 mg three times daily (300 mg/day) for patients with a creatinine clearance of at least 60 ml/min.
• The initial dose is 50 mg three times daily (150 mg/day), which may be increased to 300 mg/day within a week based on individual tolerance and effectiveness.
• For Pregabalin CR tablets, start with 165 mg once daily, increasing to 330 mg once daily within a week as needed. The maximum dose is 330 mg once daily.

✔️ Fibromyalgia Management in Adults

• The recommended daily dosage ranges from 300 to 450 mg/day.
• Treatment begins at 75 mg twice daily (150 mg/day), which may be increased to 150 mg twice daily (300 mg/day) within a week.
• If adequate relief is not achieved at 300 mg/day, the dose may be increased to 450 mg/day (225 mg twice daily).

✔️ Postherpetic Neuralgia (PHN) in Adults

• The recommended dose is 75 to 150 mg twice daily or 50 to 100 mg three times daily (totaling 150–300 mg/day) for patients with a creatinine clearance of at least 60 ml/min.
• Treatment starts at 75 mg twice daily or 50 mg three times daily (150 mg/day).
• Depending on the patient's response, the Dosage may increase to 300 mg/day within a week.
• If sufficient pain relief is not achieved after 2–4 weeks, and the patient tolerates the medication, the dose can be further increased to 600 mg/day.
• For Pregabalin CR tablets, the initial dose is 165 mg once daily, which may be increased to 330 mg/day after a week. It can be further increased to 660 mg once daily, but higher doses should only be considered for those who can tolerate 330 mg/day.

✔️ Conversion from Pregabalin Capsules to Pregabalin CR Tablets

• When transitioning from Pregabalin capsules to Pregabalin CR tablets, patients should take their regular morning Pregabalin dose and switch to Pregabalin CR with their evening meal.

✔️ Neuropathic Pain Associated with Spinal Cord Injury

• The suggested dose range is 150 to 600 mg/day.
• Treatment starts at 75 mg twice daily (150 mg/day), which may be increased to 150 mg twice daily (300 mg/day) after one week, depending on efficacy and tolerability.
• If pain relief remains insufficient after 2–3 weeks at 150 mg twice daily, the dose may be increased to 300 mg twice daily.

✔️ Children & Adolescents

• Safety and effectiveness are not established for neuropathic pain, postherpetic neuralgia, spinal cord injury-related pain, or fibromyalgia.
• Adjunctive treatment of partial-onset seizures is not approved for patients under one month of age.
• Extended-Release Tablets in Pediatrics: The safety and efficacy of Pregabalin extended-release tablets have not been confirmed in pediatric patients.

✔️ Pregabalin Capsule vs. Pregabalin CR Capsule Dose Conversion

✔️ Administration Instructions

• Pregabalin is taken orally and can be consumed with or without food.
• Pregabalin CR tablets should be taken after an evening meal.
• The tablet must be swallowed whole; do not split, crush, or chew.
• If a dose of Pregabalin CR is missed:
  • If close to bedtime, take the usual dose following a snack.
  • If missed before bedtime, take the usual dose after breakfast the next day.
  • If missed after breakfast, take the usual dose at the next scheduled evening meal.
• Discontinuation: Pregabalin should be gradually tapered over at least one week to minimize withdrawal symptoms.

✔️ Drug Interactions

• Medication Interactions: Pregabalin has a low likelihood of significant drug interactions.
• Food Interactions: No significant food-related interactions have been reported.

✔️ Contraindications

Pregabalin should not be used in individuals with a known hypersensitivity to Pregabalin or its components.

✔️ Pregnancy & Lactation

• There are no adequate human studies regarding Pregabalin use in pregnancy; expectant mothers should be informed about potential risks.
• Small amounts of Pregabalin have been found in breast milk, and due to potential risks, breastfeeding is not recommended during treatment.

✔️ Precautions & Warnings

• Angioedema (swelling in the throat, head, or neck) may occur and can cause life-threatening breathing difficulties requiring immediate medical attention. Pregabalin should be discontinued immediately in such cases.
• Allergic Reactions (hives, wheezing, difficulty breathing) require stopping the medication immediately.
• Increased Suicidal Thoughts: Antiepileptic drugs, including Pregabalin, may raise the risk of suicidal thoughts or behavior.
• Respiratory Depression: Patients with respiratory conditions or those taking CNS depressants should be closely monitored.
• Drowsiness & Dizziness: Pregabalin can impair the ability to drive or operate machinery.
• Discontinuation Risks: Stopping Pregabalin suddenly may lead to seizures or other adverse effects. It should always be tapered gradually over at least one week.
• Peripheral Edema: Caution is required when combining Pregabalin with thiazolidinedione antidiabetic agents.
