Pregabalin is a derivative of gamma-aminobutyric acid (GABA), an inhibitory neurotransmitter. However, it does not directly interact with GABA receptors (GABAA, GABAB) or benzodiazepine receptors. Instead, Pregabalin binds with high affinity to the alpha2-delta site, an auxiliary subunit of voltage-gated calcium channels in the central nervous system. While its exact mechanism of action remains unclear, studies suggest that this binding contributes to its pain-relieving and anti-seizure properties.

  • Depending on the patient's response, the Dosage may increase to 300 mg/day within a week.
  • If sufficient pain relief is not achieved after 2–4 weeks, and the patient tolerates the medication, the dose can be further increased to 600 mg/day.
  • For Pregabalin CR tablets, the initial dose is 165 mg once daily, which may be increased to 330 mg/day after a week. It can be further increased to 660 mg once daily, but higher doses should only be considered for those who can tolerate 330 mg/day.

  • When transitioning from Pregabalin capsules to Pregabalin CR tablets, patients should take their regular morning Pregabalin dose and switch to Pregabalin CR with their evening meal.
✔️ Neuropathic Pain Associated with Spinal Cord Injury
  • The suggested dose range is 150 to 600 mg/day.
  • Treatment starts at 75 mg twice daily (150 mg/day), which may be increased to 150 mg twice daily (300 mg/day) after one week, depending on efficacy and tolerability.
  • If pain relief remains insufficient after 2–3 weeks at 150 mg twice daily, the dose may be increased to 300 mg twice daily.

  • Safety and effectiveness are not established for neuropathic pain, postherpetic neuralgia, spinal cord injury-related pain, or fibromyalgia.
  • Adjunctive treatment of partial-onset seizures is not approved for patients under one month of age.
