Lower Back Pain: Causes, Exercises, Heat Therapy and Red Flags

Understand low back pain, sciatica red flags, safe hot water bag use, movement, physiotherapy and when to seek urgent medical care.

When your lower back starts hurting

A stiff lower back in the morning, pain after sitting at a desk or a sudden strain while lifting something is a familiar experience. Most uncomplicated lower back pain improves over time, but not every backache is harmless. New weakness or bladder or bowel changes need urgent attention.

Common causes of low back pain

Muscle strain, prolonged positions, reduced activity and heavy lifting are common contributors. Disc-related irritation can sometimes cause pain radiating into the buttock or leg, known as sciatica. Age-related changes on scans do not always explain pain. Less commonly, injuries, inflammatory conditions, infection or other serious illness may be involved.

Usual symptoms and warning signs

Aching, stiffness and movement-related discomfort can occur with uncomplicated mechanical back pain. New leg numbness or weakness, rapidly worsening pain, fever, unexplained weight loss, a history of cancer or significant trauma call for assessment. Leg pain alone does not prove a slipped disc.

Emergency red flags: do not wait

Go to an emergency department immediately if back pain is accompanied by new difficulty passing urine, loss of bladder or bowel control, numbness around the genitals or anus, weakness or numbness in both legs, or rapidly worsening neurological symptoms. These can signal cauda equina syndrome. Severe pain after major trauma also needs urgent assessment.

Why back pain may feel worse in winter

Some people notice more stiffness or discomfort during colder months, possibly because they move less or feel more muscle tension. There is no single proven explanation that cold weather causes all back pain by thickening joint fluid. Warmth and safe, regular movement may help with comfort.

Using a hot water bag or heating pad safely

A warm pack wrapped in a towel may give short-term relief from muscle stiffness. Use gentle warmth for roughly 15–20 minutes at a time, never directly against skin or while asleep. Check temperature carefully. If sensation or circulation is impaired, or skin is damaged, seek clinical advice before using heat. Cold packs may feel better for some recent injuries with swelling.

Keep moving within your limits

Where safe, stay as active as symptoms allow, take walking breaks and gradually return to usual activities. Prolonged bed rest can slow recovery. Avoid forcing stretches or heavy workouts that worsen radiating leg pain or neurological symptoms.

Gentle exercise and daily posture

Gentle walking or clinician-approved movements such as pelvic tilts may be useful when comfortable. Change position regularly during desk work and keep feet supported. There is no single perfect sitting posture for everyone; movement and comfort matter.

When physiotherapy, scans or medicines may be considered

Seek professional assessment if pain persists, recurs or interferes with daily life. Routine X-ray or MRI is not necessary for every episode; imaging is considered when results may change treatment. A clinician may discuss medicines, but NSAIDs carry stomach, kidney and cardiovascular risks. Mention pregnancy, other conditions and current medicines.

Back belts, massage and surgery

NICE does not recommend routine belts or corsets for low back pain. Massage may temporarily help some people but should not replace an active rehabilitation plan. A disc problem does not automatically mean surgery; specialist decisions depend on the clinical picture and neurological findings rather than a universal percentage.

When to arrange a follow-up

Arrange an appointment if symptoms are not improving over several weeks, keep returning, affect work or sleep, or new symptoms appear. Fever or unusual night pain warrants earlier review. Emergency neurological signs require immediate care, not a routine appointment.

Related products for temporary comfort

The two hot water bags and two heating pads shown below are intended for temporary comfort. They do not treat the underlying cause of sciatica or serious spinal disease. Follow product instructions and protect skin; do not delay emergency care to try a heat product.
