Pharmacology

Flunarizine is the difluorinated derivative of cinnarizine. It is a selective calcium channel antagonist. Excess calcium transmembrane influx is reduced to reduce calcium transmembrane inflow. Flunarizine prevents calcium buildup in cells. It has no effect on cellular calcium homeostasis. Flunarizine has antihistaminic and sedative properties as well. It has a 99 percent affinity for plasma protein.

Dosage & Administration of Norium 5mg Tablet
  • For Migraine Prophylaxis, the starting dose is 10 mg at night in patients under the age of 65, and 5 mg daily in patients over the age of 65.
  • If depression, extrapyramidal symptoms, or other unpleasant symptoms appear during therapy, the medication should be discontinued. Flunarizine also has calming and antihistaminic properties. It has a 99 percent affinity for plasma protein. If a patient responds well and requires maintenance therapy, the dose should be reduced to 5 days of treatment at the same daily dose with two consecutive medicine-free days each week. Even if the preventive maintenance therapy is safe and effective, It should be stopped after 6 months and resumed only if the patient requires it.
  • 10 mg twice daily, with a maximum dose of 30 mg per day if needed for peripheral vascular disease.
  • Adults should take 10-20 mg per day for vertigo and motion sickness, while children over 40 kg should take 5 mg per day.
  • Adults should take 15-20 mg daily as an add-on medication for epileptic seizures, while children should take 5 to 10 mg daily.
Interaction

Flunarizine is primarily used for the prevention and treatment of migraine headaches, as mentioned earlier. While it is not commonly associated with galactorrhea (spontaneous milk secretion from the breasts), there have been rare reports of this side effect in a small number of women taking oral contraceptives in combination with Flunarizine during the first two months of therapy. If someone experiences galactorrhea while taking Flunarizine, it is important to consult with a healthcare professional for further evaluation and guidance.

Regarding the interaction between Flunarizine and carbamazepine or phenytoin, both of these medications are hepatic enzyme inducers, meaning they can increase the metabolism of other drugs in the liver. When these medications are taken together with Flunarizine, they may enhance the metabolism of Flunarizine, potentially reducing its effectiveness. In such cases, it may be necessary to increase the dose of Flunarizine to maintain its therapeutic effect. It is essential to consult with a healthcare professional or pharmacist who can assess the specific situation and provide appropriate dosage adjustments and guidance.

Contraindications

Flunarizine hypersensitivity. Flunarizine is not recommended for those who have had a depressive episode in the past, or who have signs of Parkinson's disease or other extrapyramidal diseases.

Pregnancy & Lactation

The safety of Flunarizine in pregnancy and lactation has not been well established. Limited data is available on the use of Flunarizine during pregnancy, and it is generally recommended to avoid using Flunarizine during pregnancy unless the potential benefits outweigh the potential risks.

Similarly, the safety of Flunarizine during lactation is not well studied, and it is not recommended for use while breastfeeding. Flunarizine can pass into breast milk, and its effects on the nursing infant are not fully known.

If you are pregnant or breastfeeding or planning to become pregnant, it is essential to discuss your specific situation with your healthcare provider. They can assess the potential risks and benefits and provide guidance on alternative treatment options that may be safer during pregnancy or lactation. It is important to prioritize the health and well-being of both the mother and the baby during these stages.

Precautions & Warnings
  • Sleepiness and CNS depressants: Flunarizine can cause sleepiness as a side effect. This effect can be intensified if alcohol or other central nervous system depressants are used concurrently. It is important to exercise caution and avoid driving or engaging in activities that require mental alertness until you understand how Flunarizine affects you individually. Patients should be informed about the potential for impaired mental awareness, which could lead to accidents.
  • Not for stopping a migraine attack: Flunarizine is not recommended for treating acute migraine attacks. It is primarily used for the prevention of migraine headaches rather than providing immediate relief during an attack. Different medications or treatments may be prescribed to manage the symptoms during an acute migraine episode.
  • Extrapyramidal and depressive symptoms: Flunarizine therapy may potentially cause extrapyramidal symptoms (movement disorders) and depressive symptoms. It can also unmask or exacerbate Parkinsonism, particularly in individuals who are already susceptible to it, such as the elderly. Therefore, Flunarizine should be used with caution in such individuals. Regular monitoring and assessment of symptoms are important when using Flunarizine in these populations.
  • Precautions: Flunarizine requires special consideration in individuals with specific conditions or characteristics. This includes individuals with a history of driving or operating machinery, epilepsy, cardiovascular disease (CVS disease), and glaucoma. It is important to inform your doctor about any pre-existing medical conditions you may have, as they can provide appropriate guidance and monitor your response to the medication.
Storage Conditions
