Erectile Dysfunction is the inability to achieve and maintain a firm erection suitable for sexual intercourse. It can result from physical or psychological factors such as anxiety, depression, or lifestyle choices like smoking, excessive drinking, obesity, sedentary habits, and high blood pressure. Symptoms include difficulty in getting or sustaining an erection and reduced sexual desire.

Pulmonary Arterial Hypertension (PAH) is a condition where high blood pressure affects the arteries in the lungs and heart. This condition arises due to narrowed or obstructed blood vessels, increasing the heart's workload. Symptoms encompass chest pain, dizziness, fatigue, and swelling in the legs and ankles.

Pharmacology

Sildenafil is a type 5 phosphodiesterase inhibitor that targets cyclic guanosine monophosphate (cGMP) (PDE5). The relaxation of smooth muscle in the corpus cavern sum and its associated arterioles initiates penile erection, which is a hemodynamic process. Nitric oxide is released in the corpus cavern sum during sexual stimulation.

Dosage & Administration of Erecta 50mg Tablet

Adult Dose

Oral Erectile dysfunction:

• Adult: Take 50 mg approximately 1 hour before sexual intercourse.
• Dose adjustment may be necessary based on penile response.
• Maximum dose: 100 mg per dose, and do not take more than once in 24 hours.
• Elderly (>65 years): Start with a lower initial dose of 25 mg.
• Hepatic impairment: Initially, take 25 mg.

Pulmonary Arterial Hypertension:

• Take 20 mg three times daily, with doses administered 4-6 hours apart.

Renal Dose

For individuals with renal impairment (CrCl <30 ml/min), the recommended dosage is to start with 25 mg initially. Adjustments may be needed based on individual response and tolerance.

Interaction

• Erecta 50mg Tablet may interact with nitrate medicines (nitroglycerin), guanylate cyclase stimulator (riociguat), protease inhibitors (ritonavir), antifungals (ketoconazole, itraconazole), and alpha-blockers or antihypertensive medicines.
• Food Interactions: Taking Erecta 50mg Tablet after a heavy meal may delay its effects, so opt for lighter meals. Avoid alcohol consumption, as it can temporarily affect your ability to achieve an erection.
• Disease Interactions: Inform your doctor about any heart problems, blood circulation issues, stomach ulcers, bleeding disorders, penis deformities, or Peyronie's disease you've had or have.

Contraindications

Sildenafil is not suitable for regular smokers due to the risk of a severe eye condition known as NAION, which involves excessive blood loss in the optic nerve. It's also advised that patients who have experienced heart attacks, low blood pressure, myocardial infarction, stroke, or cardiac failure should avoid using Sildenafil to prevent serious complications. This medication is recommended for individuals with penile deformities. If you have rare inherited eye diseases like retinitis pigmentosa, Sildenafil may not be suitable. It should not be used by individuals who are hypersensitive to the drug or have been treated with medicines containing nitrates. Additionally, Sildenafil is not intended for patients under 18 years of age or those with stomach ulcers or bleeding problems like hemophilia.

Pregnancy & Lactation

Erecta 50mg Tablet is not recommended for use in women.

Precautions & Warnings

Erecta 50mg Tablet should not be taken if you have allergies to any of its components if you're using nitrates for angina or chest pain, or guanylate cyclase stimulators for heart failure or PAH. Inform your doctor about your medical history, especially if you've had heart issues like angina, heart attacks, irregular heartbeats, or heart failure. Additionally, disclose any blood circulation problems, low blood pressure, eye or ear problems, sickle cell anemia, multiple myeloma, leukemia, stomach ulcers, bleeding disorders, or conditions affecting penis shape like Peyronie's disease.
