Description:

Gelora Oral Gel is an antifungal medication used to treat various fungal or yeast infections of the skin, including athlete's foot, jock itch, candidiasis, and ringworm. These infections are caused by different types of fungi and can be spread through skin-to-skin contact. Gelora Oral Gel contains miconazole, which helps inhibit fungal growth and reduce infection.

The duration of treatment with Gelora Oral Gel may vary, but it typically takes around 4 to 7 days for symptoms to improve. It is essential to continue using the cream as prescribed by the doctor, even if the symptoms improve. Abruptly stopping the medication may lead to a relapse of the infection, where the signs and symptoms reappear. To completely eradicate the infection, follow your doctor's recommendations for the full treatment duration.

While using Gelora Oral Gel, some common side effects may occur, such as redness, rash, itching, blister formation at the application site, impaired sense of taste (dysgeusia), headache, itching or burning, abdominal pain, nausea, vomiting, and diarrhea. If these side effects persist or worsen, inform your doctor promptly. Seek immediate medical attention if you experience any other unusual symptoms.

Before using Gelora Oral Gel, inform your doctor if you are allergic to miconazole or any other antifungal agents. It's essential to disclose all medications you are taking, including anticoagulants, immunosuppressants, antifungals, antibiotics, prescription drugs, or non-prescription drugs. If you have diabetes, HIV, or AIDS, or are undergoing chemotherapy, inform your doctor before using this medicine.

Consult your doctor before using Gelora Oral Gel 15 gm in children, pregnant women, and breastfeeding mothers to ensure its safety and suitability for use in these populations. It's crucial to follow your doctor's advice and recommendations to effectively treat the fungal infection while minimizing the risk of any potential side effects or complications.

Indication OF Gelora Oral Gel

Oral treatment and prevention of fungal infections of the oropharynx and gastrointestinal tract, and of super infections due to Gram-positive bacteria.

Pharmacology

Pharmacodynamics: Miconazole exhibits antifungal activity against common dermatophytes and yeasts. It also has antibacterial action against certain gram-positive bacilli and cocci. The main mechanism of action involves the inhibition of a demethylation step in ergosterol biosynthesis, a crucial process for fungal growth. Ergosterol is an essential component of the fungal cell membrane, and its disruption leads to the formation of gaps in the cell membrane. This disruption allows essential constituents of the fungal cells to leak out, eventually causing the death of the fungal cells.

Pharmacokinetics: When administered orally, Miconazole has low bioavailability (25-30%) due to limited absorption from the intestinal tract. After administration as an oral gel, Miconazole is systemically absorbed. Once absorbed, the majority of Miconazole is bound to plasma proteins (88.2%), primarily to serum albumin and red blood cells (10.6%). The absorbed portion of Miconazole is extensively metabolized in the body. Only a very small amount (less than 1%) of the administered dose is excreted unchanged in the urine. The terminal plasma half-life of Miconazole is around 20-25 hours in most patients. The elimination half-life of Miconazole remains similar in patients with renal impairment.

Dosage & Administration of Gelora Oral Gel

The administration of Gelora Oral Gel is specific to the affected area in the mouth or throat. Here are the recommended doses for both adults and children:

Adult Dose:

  • Oropharyngeal candidiasis and Intestinal candidiasis: The usual dose is 15 mg/kg/day in divided doses. For this gel formulation, one or two teaspoons (5-10 ml) of gel should be taken four times daily. It is recommended to continue treatment for at least 1 week after the symptoms have disappeared.
  • For localized lesions of the mouth: A small amount of gel should be applied directly to the affected area with a clean finger four times daily for 5-7 days.

Child Dose:

  • Oropharyngeal candidiasis:
    • Children aged 6 years and over One teaspoonful (5 ml) of gel four times daily.
    • Children aged 2-6 years: One teaspoonful (5 ml) of gel twice daily.
    • Infants under 2 years: Half teaspoonful (2.5 ml) of gel twice daily.
    • Treatment should be continued for at least 1 week after the symptoms have disappeared.
  • For localized lesions of the mouth:
    • Smear a small amount of gel on the affected area with a clean finger four times daily for 5-7 days.

It is important to follow the prescribed dosages and treatment duration as recommended by the healthcare provider. For oropharyngeal candidiasis and intestinal candidiasis, the gel should be retained in the mouth for as long as possible before swallowing. The gel can be directly applied to localized lesions in the mouth using a clean finger. If symptoms persist or worsen, it is essential to consult a healthcare professional for further evaluation and management.

Pregnancy & Lactation

The safety of Miconazole oral gel during pregnancy has not been established through clinical studies. Therefore, it is generally recommended to avoid using Miconazole oral gel in pregnant women, especially during the first trimester when fetal development is most critical. If there is a need to use the medication during pregnancy, the potential risks should be carefully assessed and balanced against the possible benefits under the supervision of a healthcare professional.

Similarly, caution should be exercised when using Miconazole oral gel in nursing women. Since many drugs can be excreted in human breast milk, it is essential to consider the potential risks to the infant before using this medication while breastfeeding. If the use of Miconazole oral gel is deemed necessary, it should be done under the guidance of a healthcare provider to ensure the safety of both the mother and the infant.
