Pharmacodynamics:
Miconazole exhibits potent antifungal action against common dermatophytes and yeasts and also possesses antibacterial activity against some gram-positive organisms. It works by inhibiting an essential step in the production of ergosterol, a key component of the fungal cell membrane. The absence of ergosterol disrupts membrane integrity, leading to leakage of essential intracellular contents and cell death.
Pharmacokinetics:
Orally, miconazole has limited absorption (about 25–30%). When administered as an oral gel, it is partly absorbed and binds strongly to plasma proteins, mainly serum albumin and red blood cells. It undergoes hepatic metabolism, with less than 1% excreted unchanged in urine. The average elimination half-life ranges from 20 to 25 hours, with similar clearance in patients with kidney impairment.
Miconazole Cream
Rare: Nausea, vomiting, diarrhea, especially with long-term use.
Occasionally, Allergic skin reactions.
Avoid combining with terfenadine, astemizole, or cisapride, as miconazole may inhibit their metabolism and elevate toxicity risk.
May also increase blood levels of phenytoin and cyclosporine, necessitating monitoring.
Do not use in individuals with known allergy or hypersensitivity to miconazole or any component of the formulation.
