A prescription medication called Motigut Suspension is used to treat nausea, vomiting, non-ulcer dyspepsia, heartburn with or without regurgitation of gastric contents into the mouth, and indigestion. It inhibits the brain chemical that causes nausea and vomiting while also enhancing food flow in the stomach to enhance digestion. Before meals, take Motigut Suspension at the dose and time recommended by the doctor.

Pharmacology

Domperidone works as a dopamine antagonist by blocking dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and stomach. This action leads to an increase in the motility of the gastrointestinal tract. Domperidone mainly acts on the periphery and has minimal impact on the dopaminergic receptors in the brain, resulting in no psychotropic or neurological side effects. Domperidone improves the motility and tone of the upper gastrointestinal tract, facilitates gastric emptying, enhances antral and duodenal peristalsis, and regulates pyloric contraction. It also increases esophageal peristalsis and lower esophageal sphincter pressure, preventing the regurgitation of gastric contents.

Dosage & Administration of Motigut Suspension
  • The adult oral dose of domperidone for nausea and vomiting is 10-20 mg every 4-8 hours, up to a maximum of 80 mg per day. For non-ulcer dyspepsia, the dose is 10 mg 3-4 times daily, with a maximum of 20 mg 3-4 times daily. The last dose should be taken at bedtime. For migraine, the dose is 20 mg every 4 hours, up to a maximum of 4 doses in 24 hours. The rectal dose for nausea and vomiting is 60 mg twice daily. There is no need for dosage adjustment in elderly patients or those with hepatic impairment.
  • For children, the oral dose is 0.2-0.4 mg/kg every 4-8 hours daily. The rectal dose for children weighing over 15 kg but under 12 years old is 30 mg twice daily, and for those over 12 years old and over 35 kg, it is 60 mg twice daily.
  • In patients with renal impairment, no dosage adjustment is needed for mild to moderate impairment. For severe impairment, dosing should be reduced to 1-2 times daily with prolonged treatment.

Motigut Suspension can interact with various medications used to treat heart problems, high blood pressure, depression, cancer, bacterial infections, gastrointestinal disorders, malaria, neuro-related conditions, and antifungal and anti-HIV drugs. It is also recommended to avoid consuming grapefruit juice, alcohol, and tobacco while taking Motigut Suspension as they can affect its efficacy. Patients with heart, kidney, liver disease, brain tumors, a history of gastrointestinal bleeding, or any bowel-related issues should exercise caution while using Motigut Suspension.

Contraindications

Domperidone should not be used in neonatal patients and those with documented hypersensitivity to the drug. Additionally, the use of Domperidone is contraindicated in cases where gastrointestinal stimulation could be harmful, such as in cases of gastrointestinal hemorrhage, mechanical obstruction, or perforation. People with prolactin-releasing pituitary tumors (prolactinoma) should also avoid the use of Domperidone as it can stimulate prolactin release and worsen the condition.

Pregnancy & Lactation

The safety of domperidone has not been established and therefore it is not recommended during pregnancy. Animal studies have not shown any teratogenic effects on the embryo. Domperidone may stimulate galactorrhea and improve post-natal lactation. It is excreted in breast milk, but in very small amounts that are not considered harmful.
