✔️ Pharmacology
Domperidone works by selectively blocking dopamine (D₂) receptors in the chemoreceptor trigger zone and the stomach. This action enhances gastrointestinal motility, accelerates gastric emptying, increases esophageal peristalsis, and strengthens lower esophageal sphincter tone—helping prevent reflux. As domperidone poorly crosses the blood-brain barrier, it has minimal neurological side effects.

Take 15–30 minutes before meals and, if needed, before bedtime.
Adults: 10–20 mg every 6–8 hours (max 80 mg/day)
Children: 0.2–0.4 mg/kg every 6–8 hours
✔️ Interactions
CYP3A4 inhibitors (e.g., azole antifungals, macrolides, protease inhibitors, and nefazodone) may increase domperidone levels. Avoid combining with antacids or antisecretory drugs, as they reduce absorption. Domperidone can alleviate peripheral side effects of dopaminergic drugs (e.g., bromocriptine, L-DOPA) without affecting their central activity.

Generally well tolerated. Rarely, mild intestinal cramps, rash, urticaria, or transient neurological symptoms (especially in children) may occur. Elevated prolactin levels can cause galactorrhea or gynecomastia in rare cases.

- Hypersensitivity to domperidone
- Gastrointestinal hemorrhage, obstruction, or perforation
- Prolactin-releasing pituitary tumor (prolactinoma)
- Neonates

Use during pregnancy only if the benefits outweigh the potential risks. Small amounts may pass into breast milk—breastfeeding is not recommended during treatment unless necessary.
