Cholecalciferol (Vitamin D3) is a fat-soluble sterol crucial for:

✔️ Dosage & Administration
Adults:
Nutritional deficiency: 200–400 IU (5–10 mcg) daily
Osteoporosis (age >50): 800–1000 IU daily with calcium
Hypoparathyroidism: 50,000–200,000 IU daily with calcium
Can be taken orally or by intramuscular injection
Children:
0–12 months: 400 IU daily
1–18 years: 600 IU daily
Vitamin D-resistant rickets: 12,000–500,000 IU daily
Familial hypophosphatemia: 40,000–80,000 IU daily with phosphate supplements (dose may be reduced after growth completion)
Can be taken with or without food.
✔️ Side Effects
Generally well tolerated, but may cause:
Hypercalcemia or calcium toxicity
Symptoms: anorexia, dry mouth, nausea, vomiting, constipation, headache, or abdominal pain
✔️ Drug Interactions
- Increased calcium risk: With thiazide diuretics, calcium, or phosphate
- Reduced effect: With rifampicin, isoniazid, corticosteroids, ketoconazole
- Increased requirement: With antiepileptics (e.g. phenytoin, carbamazepine)
- Reduced absorption: With cholestyramine, colestipol, mineral oil, orlistat
- Caution: With digoxin or other cardiac glycosides

- Hypercalcemia
- Hypersensitivity to Vitamin D3 or excipients
- Evidence of vitamin D toxicity

Safe when used with caution
Vitamin D3 passes into breast milk; monitor both mother and infant if supplementing
✔️ Precautions
Caution is advised in patients with:
Hypercalcemia or hyperphosphatemia
Heart disease
Kidney disease
Those on magnesium or calcium therapy
Vitamin D3 must be taken with adequate calcium and magnesium to prevent secondary hyperparathyroidism and calcium loss from bones.
