Hypoparathyroidism: 50,000–200,000 IU daily with calcium; IM administration is also an option.

Children

0–12 months: 400 IU (10 mcg) daily.

1–18 years: 600 IU (15 mcg) daily.

Vitamin D–resistant rickets: 12,000–500,000 IU daily.

Familial hypophosphatemia: 40,000–80,000 IU daily with phosphate supplements; dose may be adjusted after growth is complete.

Can be taken orally or via intramuscular (IM) injection.
May be taken with or without food.

Increased hypercalcemia risk: Thiazide diuretics, calcium/phosphate supplements.
Reduced effectiveness: Rifampicin, isoniazid, ketoconazole, corticosteroids, antiepileptics (e.g., phenytoin).
Reduced absorption: Cholestyramine, colestipol, mineral oil, orlistat.
Use with caution: Digoxin or other cardiac glycosides due to arrhythmia risk.
