TAF 25mg Tablet is used for the treatment of adult patients suffering from Human Immunodeficiency Virus (HIV) infection.

Pharmacology

Tenofovir alafenamide (AF) is a nucleotide reverse transcriptase inhibitor (NRTI) and a prodrug of tenofovir. Unlike tenofovir disoproxil fumarate (tenofovir DF or Viread), tenofovir AF is a more targeted form of tenofovir. It has shown highly effective antiviral activity at a dose that is 10 times lower than tenofovir DF and offers improved safety for the kidneys and bones.

Tenofovir AF is a lipophilic compound that can easily penetrate cells. It enters primary hepatocytes through passive diffusion and by utilizing hepatic uptake transporters like OATP1B1 and OATP1B3. Once inside the cell, it is converted into tenofovir diphosphate. Tenofovir diphosphate inhibits HBV replication by becoming part of the viral DNA during the reverse transcription process, ultimately leading to DNA chain termination and stopping viral replication.

Dosage & Administration of TAF 25mg Tablet

Adult Dose

Chronic Hepatitis B Infection: TAF 25mg Tablet is indicated for the treatment of chronic hepatitis B virus (HBV) infection in adults with compensated liver disease. The recommended dose is 25 mg taken orally once daily with food.

Hepatic Impairment

  • Mild Hepatic Impairment (Child-Pugh A): No dosage adjustment is required.
  • Decompensated Hepatic Impairment (Child-Pugh B or C): Use is not recommended in patients with decompensated hepatic impairment.

Renal Dose

  • Renal Impairment (Mild, Moderate, or Severe): No dosage adjustment is required.
  • End-Stage Renal Disease (CrCl <15 mL/min): Use is not recommended.

Take TAF 25mg Tablet as prescribed by your physician. Swallow the tablet with a glass of water. Your doctor will determine the appropriate dose and duration based on your age, body weight, and the severity of your disease. Do not discontinue TAF 25mg Tablet unless advised to do so by your doctor.

Interaction

However, animal studies did not show any adverse effects on fetal development when tenofovir alafenamide was administered during the period of organogenesis. These studies were conducted at exposure levels equal to or 51 times higher than the tenofovir alafenamide exposure at the recommended daily dose in both rats and rabbits.

  • Lactic Acidosis and Hepatomegaly: Lactic acidosis (excess lactic acid in the blood) and severe hepatomegaly (enlarged liver with fat accumulation) have been reported with the use of nucleoside analogs, including tenofovir disoproxil fumarate, especially when used in combination with other antiretroviral drugs. Most of these cases were reported in women, and risk factors may include obesity and prolonged exposure to nucleoside analogs.
  • Hepatitis B Exacerbation: Discontinuation of antihepatitis B drugs, including tenofovir alafenamide, can lead to severe acute exacerbations of hepatitis B. Therefore, treatment should not be abruptly stopped without medical supervision.
  • HIV-1 Resistance: Tenofovir alafenamide alone is not recommended for the treatment of HIV-1 infection. Before initiating treatment, patients should be tested for HIV-1. Using tenofovir alafenamide alone for HIV-1 treatment can lead to the development of HIV-1 resistance.
