Linezolid is a synthetic antibacterial agent belonging to the oxazolidinone class. It is primarily active against Gram-positive aerobic bacteria, certain Gram-positive anaerobic organisms, and selected bacteria under laboratory conditions.
It works through a mechanism that differs from most other antibiotics. Linezolid binds to the 23S ribosomal RNA located within the 50S subunit of the bacterial ribosome. This prevents formation of the functional 70S initiation complex, an essential step in bacterial protein translation.
By blocking bacterial protein synthesis, linezolid prevents bacteria from producing proteins required for growth and survival. It is generally bacteriostatic against enterococci and staphylococci, while it may demonstrate bactericidal activity against many susceptible streptococcal strains.
Linezolid tablets may be taken orally with or without food. When given intravenously, the infusion should be administered over 30–120 minutes.

Neonates younger than 7 days:
Most premature neonates younger than 7 days, particularly those born at a gestational age below 34 weeks, may have slower clearance of linezolid and greater drug exposure. A starting regimen of 10 mg/kg every 12 hours may be considered. If clinical response is inadequate, the frequency may be increased to every 8 hours under specialist supervision. By 7 days of age, neonatal patients should generally receive 10 mg/kg every 8 hours.
No dosage adjustment is required when changing from intravenous to oral linezolid, as the oral formulation provides comparable systemic exposure.
Pregnancy:
Available clinical data have not demonstrated a clear association between linezolid exposure during pregnancy and major birth defects, miscarriage, or adverse maternal or fetal outcomes. Nevertheless, Lizox should be used during pregnancy only when the expected benefit outweighs the potential risk to the fetus.
Breastfeeding:
Linezolid passes into breast milk. The benefits of breastfeeding, the mother's need for linezolid, and potential effects on the breastfed infant should all be considered. Breastfed infants should be monitored for diarrhea and vomiting.
Linezolid is a reversible, nonselective monoamine oxidase (MAO) inhibitor and may therefore interact with adrenergic and serotonergic medicines.
Adrenergic, vasopressor and dopaminergic medicines:
Linezolid may enhance the blood-pressure-raising effect of indirect sympathomimetics, vasopressors, or dopaminergic agents. Close blood-pressure monitoring is required when such combinations are considered.
Serotonergic medicines:
The combination with serotonin reuptake inhibitors, tricyclic antidepressants, triptans, meperidine, buspirone, and other serotonergic agents may increase the risk of serotonin syndrome. Patients should be monitored carefully for relevant symptoms.
Food interactions:
Avoid consuming large quantities of tyramine-rich foods or beverages while taking linezolid, as excessive tyramine intake may contribute to increased blood pressure.
